Completion of this application will provide your company the opportunity to pre-qualify with the City of Jacksonville Beach, Florida. This will allow your company to be considered for eligibility to bid on this project. However, completing this application does not guarantee your company will receive a determination that it is “Qualified to Bid” or a contract from the City, nor does it imply that your company has any type of procurement/contractual relationship with the City now or in the future. Information provided by your company will be treated in a reasonable manner and will be subject to disclosure pursuant to the laws of the State of Florida. Each contractor who submits a Pre-Qualification Application package will receive from the City a “NoticeForm-Determination of Whether Qualified or Not Qualified to Bid” on this project. Only those contractors who receive a Notice Form marked “Qualified to Bid” from the City will be allowed to submit bids on this project.

The City of Jacksonville Beach is working to enhance the overall community quality of life of its Downtown District.ThePhase IIIC – Project 2area includes 11th Avenue South to 13th Avenue South and 3rd Street South (A1A) to the Endzone, 12th Avenue South from 3rd Street to the Central Basin at 4th Street South, 4th Street South from 10th Avenue South to 12th Avenue South, and stormwater pumping station improvements located at Beach Boulevard and 2nd Street North.

GENERAL WORK SUMMARY

Phase IIIC – Project 2 Infrastructure Improvements is a complete roadway reconstruction project. The project includes demolition, relocation and reconstruction of all sidewalks including pavers, construction of additional on-street concrete parking bays, new storm sewer piping, inletsand associated work, replacement of stormwater pumps and motors, replacement of valves, painting of piping and valves, new flowmeter and vault, new concrete energy dissipator at existing golf course pond, structural modifications to stormwater vault, building modifications, new electrical equipment, new generator, new diesel fuel tank, natural gas supply for generator, new water mains and associated work, new sanitary sewer mainsand associated work, coordination of third party utility adjustments, new/relocated light poles, new traffic control signage, adjustments to electrical junction/pull boxes, landscaping, landscaping curbing, replacement of fire hydrants, replacement of driveway aprons, coordination with property owners, maintenance of business and pedestrian access, access for delivery and solid waste collectionvehicles, and removal of shrubs and trees within right-of-way.

The project shall be constructed in multiples stages to allow for minimal resident, business and traffic disruption. The contractor shall maintain essential services (water, sewer, electric & garbage pickup), to the extent possible, throughout construction. The contractor shall provide maintenance of traffic throughout construction. The contractor will be expected to follow all applicable rules, laws and regulations as well as provide all necessary permits for the work.

REQUIRED INFORMATION

1. Firm Background:

A.Firm’s description, office location, time in business, background and references.

B.Firm’s experience in the State of Florida.

C.Provide license documentationto do business in the State of Floridaand to meet all state and local regulations. Provide copies of all documentation (licenses, etc.) with submittal.

D.Current Financial Statement completed within the past year.

2. Firm Qualifications:

A.Team Organization: Identify specific personnel that will be assigned to the project, including: Project Manager, Project Superintendent, and lead foreman. Personnel identified in this Pre-Qualification Applicationshall be the principal staff that will work on the project. Project staff must meet all local, state and federal requirements to perform work.

B.Provide a list of multiple sub-contractors that you intend to utilize on this project and the type of work they will perform.

C.Firm shall show that it has the resources to complete a minimum of 30% of the total construction work, estimated between $5 million and $7 million, using company staff and company owned/leased equipment.

3. Completed Projects:

  1. Provide three (3) similar projects your firm has successfully constructed within the last fifteen (15) years. Similar projects are defined as urban reconstruction projects, commercial or residential, with a final construction cost of at least $500,000.
  2. Provide one (1) similar water, sewer, or stormwater master pumping station your firm or your subcontractor has successfully constructed within the last fifteen (15) years.
  3. List the following information for each project:
  4. Project reference including name of client/owner, name, address, and telephone number.
  5. Work description
  6. Original contract amount and actual completed amount.
  7. Contract completion date and actual completion date including details of any incentive payments your firm received for early completion or penalties your firm was assessed.

4. General Approach to Project:

A.Provide an overview of how you plan to maintain pedestrian and vehicular trafficin the work area.

B.Provide an overview of how you plan to maintain business and private property access for the duration of the project.

C.Detail how you will keep businesses, property owners and residents informed of the construction progress and potential service disruptions.

D.Provide the name of the person who will be responsible for daily interaction and communication with the City, businesses, residents and property owners.

E.Provide a general phasing plan outlining how you anticipate constructing the project within the 18-month duration.

5. Miscellaneous Project Information:

  1. Contractors are required to purchase a CD of the draft copy of the project documentsat a cost of $30, from the Project Engineer’s office, GAI Consultants, Inc., located at 1301 Riverplace Blvd., Suite 900, Jacksonville, Florida 32207. Contact Mr. Matt Bolyard, P.E. at 904-559-8086 or 904-627-5549. Failure to purchase a draft copy of the project documents will result in disqualification.

6. Bid Advertisement Date:December 21, 2016.

7. Florida Contractor’s License: Firm must have a valid Florida GeneralContractor’s license and/or a valid Florida Underground Utility Contractor’s license to perform the construction of the project.

8. Bankruptcy: No firm will be authorized to bid on this CITY project or be issued a contract for work, which has filed for bankruptcy in the past seven (7) years and that no owner/officer or principal of the aforementioned firm has filed for bankruptcy personally in the past seven (7) years or has been an owner/officer or principal of a firm which has filed for bankruptcy in the past seven (7) years.

9. Notice Form-Determination of Whether Qualified or Not Qualified to Bid: Each submitted pre-qualification application must include this notice form so that the applicant firm can be notified of the Project Engineer’s determination of whether qualified or not qualified to bid. Contractor should receive this notification on February 9, 2017.
