Powder Puff Game Registration Packet
For Players and Coaches
* You must have all necessary items in order to register, soread the entire packet.
* Only Juniors and Seniors may register.
* Players will be selected from eligible candidates through a lottery system.
* There will be a LIMIT OF 25 PLAYERS and 5 COACHES per team.

Beforeyouregister,be awareofthefollowing:
You must submit the completed registration packet to Mrs. Hogentogler (IPC-2C) by October 5 at 2:35pm. Late packets will not be accepted for any reason.
Teams will be posted bythe end of the day on October 9 outside of IPC-2C.
If you are selected in the lottery, you must give a check payable to Central Dauphin Four Diamonds Committeefor $50 to Mrs. Hogentogler by 2:35 pm on October 12. Please address the check as specified or it will be returned. Also, please write the name of the player or coach on the memo line. Remember, the $50 takes care of the entrance fee to the mini-thon dance and guarantees a t-shirt for you. If you would like to order a photo CD of the game pictures, please include the $5.00 fee and make the check for $55.
You must submit a completed Academic Eligibility form. Each participant must have an average of 60% or higher in all classes. Falling below 60% will make you ineligible to play in the game.
You must provide insurance coverage information with parent signature.
You must turn in a participant rules contract signed by both student and parent.
You must attend a mandatory meeting onOctober 12, 2012 at 2:35 pm in the cafeteria.
You must attend both scheduled practices.

Hazing is not permitted and will result in removal from the team and disciplinary/legal action. Offenses that occur both on and off school grounds will be punished equally.
The game will take place on Tuesday, November 6, 2012 at 7:00 pm. A
rain date of Thursday, November 8, has been established.
Practices will take place on Saturday,October 13 from 11:00 am to 12:00 pm and Saturday, October 20 from 11:00 am to 12:00 pm at Landis Field. Players must attend each practice and must arrive on time. In the event of inclement weather, please check Mrs. Hogentogler’s school website for information under the “Powder Puff” link.
Participants must attend all practices—attendance will be taken. Not showing up for practice will result in an individual’s name being removed from the team roster.
Coaches/Players may not hold practice on school property without teacher supervision.
Team coaches are responsible for playing each member of the team—no exceptions.
Unnecessary roughness during the game will result in a 15-yard penalty for the team and ejection from the remainder of the game for the offender.
In order to participate in Powder Puff Football, every student must be in good
disciplinary standing with the school; any disciplinary infraction prior to the Powder
Puff Football game may result in expulsion from Powder Puff participation.
Allplayersandcoaches are subject to the Student Code of Conduct during both the practices and the Powder Puff Football game; violationoftheStudentCodeofConduct will result in disciplinary actionbeyond expulsion from the event.
Player and coach lists will be confirmed during an announced meeting that will be held a
week or so after registration is completed. In order to meet eligibility, all participants must
have turned in a signed Participant Rules Contract, a parental consent form with details of
insurance, an academic eligibility sheet, and a check in the amount of the designated
donation.
AT THE END OF THE POWDER PUFF FOOTBALL GAME, ALL PARTICIPANTS WILL RETURN FLAGS, FOOTBALLS, AND JERSEYS. NO ONE MAY LEAVE THE FIELD UNTIL ALL ITEMS ARE RETURNED.

Your son/daughter has indicated an interest in participating in annual Four Diamonds' Powder Puff footballgame. All participants must have medical insurance coverage to be eligible to participate. Central Dauphin School District does not assume liability for accidents or injuries sustained during practices or during the game. Please be sure to review and sign the attached Participant Rules and Contract form as well. Your son/daughter must turn in these forms at the time of registration.
