PORTFOLIO GENERAL PRACTITIONER

Full Time

Salary Scale - £69,698 - £80,364 per annum

Applications are invited to join a team of Portfolio GPs based within NHS Forth Valley Community Services Directorate. This innovative post offers the postholder a key role in delivering community based patient care within Community Hospital, General Practice, and the Substance Misuse Service.

In addition to providing general medical services, the postholder will be supported and enabled to develop additional expertise in specific healthcare settings and with specialist services. The Portfolio GP will also have the opportunity to participate in the redesign of care pathways for wider community based models of care.

The job plan for the portfolio role will include working within NHS Forth Valley’s Community Hospital inpatient wards which will be arranged in conjunction with a retained session/s with a General Practice. In addition, the postholder will work two days/week in the Substance Misuse Service. The Portfolio GPs will work within a multi-disciplinary team of healthcare professionals for all elements of work undertaken.

The successful candidate will be a key partner in helping to audit and evaluate the current model of care with an aim to support the development of future guidelines and improved ways of working, linked with the evolving model of inpatient care within Forth Valley. As well as the post providing an opportunity to develop your own skills, a key role will be to support and provide mentorship within multi-disciplinary teams allowing the development of extended roles in areas such as Independent Prescribing and Advanced Clinical Assessment.

As an experienced primary care practitioner with a firm grasp of issues surrounding Community Hospital Inpatient Services, you will require to hold a Certificate of Prescribed Experience from JCPTGP or equivalent and be eligible for the NHS Forth Valley Performers List. RCGP qualification is preferred.

Ability and willingness to travel throughout the area of service deliveryis essential for this post.

There is no requirement for out of hours work. Applications for job share/part-time working are welcome.

Job Title: Portfolio General Practitioner

Professionally Responsible to: CSD Clinical Lead

Operationally Responsible to: Clinical Nurse Manager

Background

The Portfolio General Practitioner will play a key role in delivering community based patient care both within general practice and across a range of community settings in Forth Valley. The postholder will provide general medical services and be supported and encouraged to develop expertise in specific healthcare settings and services. The postholder will have up to three separate care elements within their portfolio. These will be a retained role within a general practice, specific input into community hospital inpatient wards and work within the Substance Misuse Service. In all settings the postholder will deliver patient care as part of an experienced multidisciplinary team.

2.  Job Purpose

The Portfolio GP will:

·  In all settings work as part of a multi-disciplinary team of professional and administrative colleagues to provide a range of appropriate services to patients and with others will ensure optimal and well managed team performance.

·  In all settings, provide consultation and appropriate clinical management while maintaining effective communication and networking with other team members

·  Keep contemporaneous, accurate and legible records of all patient contacts and contribute to electronic data recording as directed by the clinical lead and clinical nurse manager

·  Have responsibility for personal CPD and developing and maintaining the skills required to deliver high quality medical care

·  Have a key role in clinical governance including participating in shaping future service development through guideline development and implementation as well as participating in audit and developing and initiating new systems of working in consultation with the clinical lead and clinical nurse manager.

·  Play a key role in the development and mentoring of other practitioners in the wards, particularly nursing colleagues

·  Have a key role in influencing the strategic development of Services.

3. Key Result Areas

·  As part of a core team of Portfolio GPs, provide high quality day to day medical input to patients within each of the portfolio settings.

·  Work within a specified General Practice for an agreed number of session within the NHS Forth Valley Health Board area.

·  Provide day to day medical assessment and prescribe general medical care for patients as required, depending on their medical condition.

·  Actively support the multidisciplinary team in delivery of high quality, “best practice” care focussed around the needs of patients, particularly:

Ø  Supporting prompt, safe and effective discharge, including completion of the electronic discharge summary and liaison with other primary care colleagues as appropriate.

Ø  Take a proactive approach to anticipatory care planning

Ø  To promote best practice and work to relevant locally and nationally agreed pathways and protocols. This should include:

·  End of life palliative care

·  Complex Care of the Elderly

·  Rehabilitation (may include stroke and acquired brain injury)

·  Old Age Psychiatry

Best practice should reflect up to date practice guidance appropriate to the models of care for community wards.

·  Provide first medical point of contact (in hours) when a patient becomes unwell and requires medical input.

·  Support the evolving model of inpatient care with all efforts aimed at evolving a sustainable model with a multidisciplinary team that is enabled and empowered to manage most day to day care and decisions.

·  Provide mentoring and support for other non-medical colleagues to allow development and delivery of enhanced roles such as prescribing and advanced clinical assessment.

·  Maintain a close working relationship with the multidisciplinary team including the relevant Consultant for each ward and support appropriate multidisciplinary team meetings and ward/departmental case conferences when appropriate.

·  Pursue continuing professional and personal development.

4. Assignment and Review of Work

The Portfolio GP will work closely with relevant senior clinical and managerial staff (such as the GP practice lead, CSD clinical lead, Ageing and Health Consultant, Clinical Nurse Manager) to agree specific requirements for appropriate management of the work of the team.

6. Qualification and/or Experience specified for the post

Certificate of Prescribed Experience from JCPTGP or equivalent.

An experienced primary care practitioner with a firm grasp of issues surrounding integrated healthcare systems and knowledge of Community Hospital inpatient services. (or substitute other job area depending on portfolio)

Self motivated, able to work without supervision, using own initiative to complete tasks.

Able to communicate effectively and work as part of a multi-disciplinary team.

Well developed influencing skills combined with a facilitative and supportive style of working.

RCGP qualification preferred.

Previous experience of working with Older People preferred.

Recent evidence of continued academic and professional development.

Requires to be included on or eligible for inclusion on the NHS Forth Valley GP Performers List
