REQUIREMENTS FOR ROTATING RESIDENTS & FELLOWS

Thank you for your interest in a rotation at John Stroger Hospital. Below is a summary of what is required in order to begin your rotation and obtain your ID badge, computer sign-on and pharmacy privileges. You cannot begin your rotation until you have met all of the requirements and cleared through our office.

STEP 1: Electronic Application

·  Must complete and submit the electronic application (found on our website) within 90 days prior to the start of the rotation (submitting an electronic application is an annual requirement for those who are returning). Go to Education and Research, link to rotating residents and fellows, scroll past the pictures and you will find the application. There is no separate link for the application. If you can’t see it, you may have to try different computer, not all will display it.

STEP 2: Computer Training (PowerChart Class)

·  Must attend computer training (POWERCHART in-person) within 90 days prior to the start of the rotation (this is a one time requirement). You will receive a link to sign-up for a class immediately after submitting your electronic application. Wait for the link! You must attend this class before visiting our office to start your rotation. You will NOT be allowed to start your rotation if you have not attended computer training through our institution.

STEP 3: Online Educational Modules

·  Must complete the on-line educational modules. You will receive a link after submitting your electronic application. As proof, please print the Infection Control certificate and the email from us with your passing score for the Rotators Orientation test (we grade it and email you the score-passing 8/8 or 100%). Educational modules must be retaken every 12 months through our website.

STEP 4: Check-In Process

After you have completed all of the requirements above, please visit our office (Hektoen Building, 627 S. Wood St.-8Th FL RM #831) to go through the check-in process: NOTE: We cannot give you paperwork to obtain an ID, computer log-in (2 business days to activate), etc. until 2 business days before the start of your rotation (this is a Human Resources policy and cannot be changed). Please be aware that it takes exactly 2 business days for the computer access to be activated.

·  During the check-in process we will give you the appropriate paperwork to get your ID badge, computer sign-on (2 business days to activate) and pharmacy privileges. Human Resources issues IDs between the hours of 8:30 AM - 3:00 PM, therefore please try to visit our office within that timeframe.

Required documents that all interns, residents and fellows rotating at Stroger need to bring with to our office NO SOONER than 2 business days (Human Resources Policy) prior to starting the rotation are:

You must bring hard copies of the documents with you, DO NOT fax or email ahead of time!

ü  Copy of your current medical license

ü  Copy of your current ACLS card (PGY2’s and above only). If expired, you must provide documentation that you have registered for a class.

ü  Copy of professional liability insurance if your residency does not have a standing agreement with Stroger Hospital

ü  Certificate of compliance health form/quantiferon gold (current PPD, titers, immunizations, documentation of flu vaccine effective Oct 15th)

ü  Copy of Medicare registration

ü  National Provider Identification Number (please have your NPI# with you)

Please note that computers are not available in our office for use to print the above documents, therefore please bring paper copies of all necessary documents with you. On your phone is not acceptable, must be a paper copy of all documents. Have your NPI# ready.

PLEASE UNDERSTAND THAT WE CANNOT ISSUE YOU AN ID BADGE, COMPUTER ACCESS AND PHARMACY PRIVILEGES IF YOU HAVE NOT MET ALL OF THE ABOVE REQUIREMENTS.
