Position Title: Case Worker

Reports to: Manager

Working Relationship: Manager

Rate of Pay: SW Class 2. Other terms and conditions per SCHADS (Social, Community, Home Care and Disability Services Awards 2010)

Hours of Work: 7 hours per week

Key Responsibilities:

1.  Client Services

-  Participate in developing assessment and referral processes for clients presenting in the Information Services with complex needs.

-  Provide general casework to clients to facilitate access to information and to build skills and capabilities which improve their choices.

-  Provide general case coordination services to clients with complex needs, particularly those who are homeless or at rick of homelessness.

-  Establish and implement client service data collection system and evaluation survey to meet reporting requirements and contribute to quality improvement.

Program Objectives:

This position is a specialist role to provide support clients with complex presentations, including those with mental health issues who attend MADDV for emergency relief. This role will provide both an agency based and outreach service as required.

Clients

-  Identify andy engage these clients when they come into MADDV seeking emergency relief

-  Provide assessment, immediate and longer term needs, responding to clients in a timely and professional manner.

-  Utilize a full range of assertive outreach strategies in order to engage with individuals who may be reluctant to receive the service.

-  Work within a client empowerment model using a strengths based approach

-  Provide information, referral and assistance to clients, working with individuals to improve their financial independence, community connections and access to appropriate community resources and services.

-  Undertake casework on the basis of an agreed action plan to address identified issues

Program

-  Undertake administrative duties associated with service delivery, ensuring that agency and program requirements regarding case records, data collection and reports are fulfilled.

-  Participate in other duties as directed.

Selection Criteria:

Qualifications

-  Tertiary qualification on Social Work, or other related field

Knowledge and skills

-  Experience working with vulnerable members of the community including those from CALD backgrounds

-  A strong commitment to social justice, a sensitivity to and demonstrated understanding of the issues of crisis, financial stress and homelessness or potential homelessness and the impact on people experiencing these.

-  Case work experience in community organisation or related sector

-  Effective interpersonal, cross cultural communication, advocacy skills and the ability to liaise effectively with funding bodies, partners and other services.

-  Demonstrated experience in working autonomously as well as collaboratively with paid staff and volunteers

-  Strong written communication, report writing, computer, organisational and time management skills

-  Demonstrated positive values of integrity, reliability, teamwork and professionalism consistent with MADDV’s mission and demonstrated commitment to performance management, quality and continuous improvement principles

Other Relevant Information

-  Appointment is subject to satisfactory Police Check and signing the MADDV Code of Ethics

-  Working with children’s check is required

-  A three month probationary period applies to the position
