Position Summary / This position is responsible for the effective coordination of the processes and procedures as they relate to the patient journey from pre admission to discharge.
Ensuring individual patient engagement in their care plan and preparation for ongoing care as appropriate.
Mandatory qualifications/ criteria / ·  Current registration with the Nursing Board of Australia, AHPRA.
·  Minimum three years post graduate experience including experience in a mental health setting
Desirable qualifications/ experience / ·  Possession of or working towards post graduate qualification in mental health nursing
·  Current knowledge of legislation and clinical standards relevant to the mental health setting
·  Previous experience in managing admission and discharge procedures in the mental health setting.
Personal qualities Required / ·  Demonstrated high level interpersonal and communication skills and the ability to facilitate change in management and practice
·  Ability to effectively work within a team.
·  Ability to set goals, formulate and implement plans to reach identified outcomes.
·  To present professional and ethical work standards at all times
Professional qualities Required / ·  Annual completion of Mandatory Training
·  Regular participation in professional forums and meetings.
·  Demonstrated competence in physical and mental health assessment and evaluation of mental health patients.
·  Knowledge of Townsville Private Clinic Healthe Care policies and procedures.
·  Ability to effectively work within a multi disciplinary team.
·  Ability to change and adapt to new models of care and changing clinical practice based on evidence and research
·  Ability to manage changes in process and practice whilst maintaining a harmonious work environment.
·  Understanding the funding requirements for insured and self funded patients when seeking admission to the Townsville Private Clinic
Key Responsibility 1 / Coordinate the appropriate admission and effective discharge of inpatients
Performance Criteria 1 / ·  Establish plan for review/ development of patient flow management policies, procedures and systems for their continuous monitoring and improvement.
·  Monitor and manage bed utilisation through participation in operational and strategic planning discussions that include current and future bed demand and patient flow.
·  Provide professional guidance, leadership and support to nursing, medical and other professionals in the principles of effective patient flow processes and decision making.
·  Actively seek patient input into their treatment plan at admission and throughout their stay
·  In conjunction with multidisciplinary team proactively identify patients for discharge or transfer to ensure appropriate, timely discharge of patients and maximum utilisation of beds.
·  Participate in the ongoing review of admission documentation/ packs that provide patients and their carers with all relevant information re Clinic practices and facilities. Including rights and responsibilities
Key Responsibility 2 / Maintain Clinical Standards
Performance Criteria 2 / ·  Participate in the ACHS accreditation process and regulatory licensing requirements
·  Promote a culture of evidence based practice in clinical settings, including innovative models of care.
·  Ensure uniform standards and practice in patient flow management as it applies to nursing standards and facilitate the conduct of quality assurance and risk management activities in all relevant areas.
·  Investigate complaints as they relate to patient flow management and initiate corrective actions where this may be required in collaboration with the NUM and Quality Manager
·  To undertake audits to benchmark and evidence progress in practise appropriate to the role.
Key Responsibility 3 / Resource Management
Performance Criteria 3 / ·  Educate and empower ward staff to expedite all admissions & discharges within the operational time frames
·  Identify opportunities for career succession planning for staff within the Clinic for patient flow management.
·  To identify areas of practice where patient throughput can be improved and proactively together with the NUM engage staff to participate in change process
·  Provide advice and support to the Executive Management Team on ways to enhance performance and general productivity measures using patient flow management strategies.
Key Responsibility 4 / Actively engage patients and families in their care
Performance Criteria 4 / ·  Ensure all patient flow policies and procedures include the need to actively involve patient’s and their carers
·  Regularly seek feedback from patients and the Consumer and Carer Committee re the satisfaction with all aspects of admission and discharge practices. Include the readability & relevance of verbal and written information provided.
·  Respond promptly to complaints and compliments. Feedback to consumers any proposed changes resulting from their input.

5 / Multidisciplinary Team

Criteria 5 / ·  Establish and maintain collaborative working arrangements with the multi-disciplinary team and other community service providers.
·  As part of the multi-disciplinary team, participate in planning and review of patient care and discharge arrangements.

6 / Quality Improvement
Performance Criteria 6 / ·  Participates in the quality development, implementation and evaluation of clinical service delivery policy, procedures, programs and systems.
·  Attend regular staff meetings, and when required serve on relevant committees.
·  Active participation in quality improvement activities and risk management activities within the Clinic. Such activities should meet the requirements of the ACHS Evaluation and Quality Improvement Program (EquIP).
·  Completion and documentation of at least 2 Quality activities per annum
·  Active participation in the Occupational Health and Safety and Infection Control programs to ensure a safe environment is provided in accordance with relevant legislation, regulations and codes of practice.
·  Contributes to program development and initiative.
·  Participates in nursing policy review and initiatives.

Criteria 7 / ·  Participates in own performance review and development.
·  Identifies own training needs.
Participates in clinical supervision of nursing staff..

Probation Requirements / All new permanent employees to the Townsville Private Clinic will be required to undertake a period of probation upon commencement of duty. This period will be 6 months in duration with a possible 3 month extension if performance objectives are not met.
