THE ROLE

Under the direction of the Manager, Positive Behaviour Support, the Personal Planner will assist people with a disability and their family and carers to consider the option of services from the People with Disability division at Rise, focussing on marketing and business development. People who choose to commence serviceswill be supported through a comprehensive intake process with a view to co-designing a high quality and individualised service. The Personal Planner will work in partnership with Team Leaders and staff from across the division to ensure the information collected and plans developed is seamlessly transferred to the staff and teams who will provide ongoing support. The Personal Planner will work in partnership with People with Disability Management and Rise Marketing team to develop and present materials that promote Rise as a quality provider of services and supports. The role is also responsible for direct marketing to a variety of referral pathways and completing funding plans and new business. The Personal Planner will undertake an independent post intake audit to assess the delivery of services against the documented plan to ensure they are meeting the needs of the person.

KEY RESULT AREAS

Area 1:
Marketing, PromotionNew Business / Expected Deliverables Include:
  1. Liaise with the People with Disability Managers and Marketing & Communications team to develop a Marketing and Promotion Plan for the division.
  2. Liaise with key stakeholders to develop divisional promotion strategies and materials that profile Rise in a positive way.
  3. Coordinate and contribute to content that profiles the division and Rise via newsletters, website, social media and other approved communication tools.
  4. Liaise with the People with Disability Managers to develop targets relating to the number of people and income per program area and translate this into funded services.
  5. Monthly monitoring and recording of key targets relating to the Marketing and Promotion Plan.

Area 2:
Program Development
Quality Audit / Expected Deliverables Include:
  1. Engage with the people we support (and their natural supports) to understand and document their abilities and needs, utilising person centred planning, positive behaviour support and other documentation relevant to the person.
  2. Collect evidential records and other relevant documentation required for referral, registration and commencement of services.
  3. Research and prepare funding plans, and alternate applications for funding, that meet the individual needs of the people we support.
  4. Engage with relevant Team Leader(s) and Manager for the safe and seamless transition into planned services.
  5. Inform new people of their rights and responsibilities and ensure that this is recorded.
  6. Act as an advocate to ensure that services are in line with the stated (and recorded) needs of the people we support.
  7. Undertake a post intake review to evaluate service delivery against stated service and outcome plans (100 day review).
  8. Provide written and/or verbal reports as requested by the Behaviour Support Manager

Area 2:
Relationship Management / Expected Deliverables Include:
  1. Develop and maintain open and effective communication to foster and promote a culture of Family and Person Centred services.
  2. Develop and maintain effective communication and collaboration with Program Managers, Team Leaders and support and administration staff.
  3. Establish and maintain professional relationships with complementary services, government agencies and other sources of referrals for people we support.
  4. Represent Rise on committees, working parties, forums, seminars and attend meetings, acting in the best interests of the organisation.
  5. Represent Rise in the community and at organisationalfunctions.
  6. Maintain productive and positive relationships with key stakeholders identified in the Marketing and Promotion Plan to enhance profile and increase funded services.

Area 4:
Projects / Expected Deliverables Include:
  1. Assist in the preparation of grants and funding applications.
  2. Participate in internal audits measured by the Disability Service Standards and division service standards to promote quality services and a culture ofcontinuous improvement.
  3. Research and recommend appropriate tools for quantitative and qualitative feedback measures to assist with the regular evaluation of service delivery.
  4. Collate feedback and evaluation results to contribute to the overall profile of the division.
  5. Other duties as required.

  2. Enact and promote Rise’s values and ethos in all behaviour, relationships and communications.
  3. Fulfil employee requirements under Section 20 of the Occupational Safety and Health Act.

Disability Services Standards /
  1. Maintain a professional manner at all times promoting a positive image of the people we support and the values of Rise.
  2. Model Disability Service Standards to staff and ensure any breaches of the Standards are reported through line management

SELECTION CRITERIA

This position requires a detailed knowledge and understanding of the policies, practices and statutory requirements of work within this area.Prerequisites include:

Qualifications and Experience:

  • Relevant tertiary degree and/or considerable experience in a similar or related field of work.
  • Strong values base on the abilities and strengths of people with disabilities.
  • Knowledge of the practical issues affecting people with a disability and the staff who support them.
  • Demonstrated ability to engage with people with disabilities and significant people in their lives in a positive and productive manner.
  • Demonstrated experience in marketing, promotion and the generation of new business.

Skills and Knowledge:

  • Demonstrated negotiation and advocacy skills
  • Ability to work both as part of a team and with a high level of independence and autonomy when required.
  • Ability to record and compile documentation to a high standard
  • Highly developed interpersonal and written communication skills
  • High level organisation and time management skills
  • Computer literate – Microsoft Office Suite
