PERFORMERS APPLICATION FORM
Before completing the application form, you should carefully read the Performer's Terms and Conditions for performing at Magdalen Street Celebration 2015 (MSC15) so that you fully understand the basis on which, should your application be accepted, you would be participating in MSC15.
The Performer’s Terms & Conditions also contains useful information that successful applicants will need for participating on the day, therefore we suggest that you print off or store a copy of the Performer’s Terms & Conditions in a safe place.
Application forms must be submitted by no later than 20th August 2015 at 6 pm. Successful applicants will be contacted as soon as possible after this date.

Insurance
Please provide evidence of your Public Liability Insurance with your completed form. Alternatively, submit a £ 10 fee by BACS (to "Magdalen Street Celebration"), or via Cheque (payable to Magdalen Street Celebration). Payments may be sent to MAC 2015, 52 St. Augustines Street, Norwich, NR3 3AD.
Promotional Material
Successful performers who return their application form before Thursday 23rd July 2015 may be mentioned in promotional material for MSC15. Please email a maximum of 2 high resolution photographs that we can use for this purpose. We will also be taking photos during the event that may be used for promotional purposes.

Acceptance of terms and conditions and data protection
The Magdalen Street Celebration will collect and process the information you provide in your application form (and in any subsequent correspondence) for the purpose of managing and facilitating your attendance at MSC15 only. The Magdalen Street Celebration will process your information in accordance with the Data Protection Act 1998 and will not share your personal information with third parties except where required in order to manage and facilitate your attendance at the event. All performers must agree to MSC15’s Performer Terms and Conditions.
