Scenario /
- Jane Smith is a 40 year old female.
- Jane’s son, Junior, is 10 years old and has autism.
- Jane keeps track of Junior’s health history, his medications, and his day-to-day progress toward the care plan that was established by his physician.
- Each day, Jane notes Junior’s progress against his care plan, and records any changes which could be associated with medication changes or developmental changes.
- Junior has a sitter who stays with him in the afternoon between the time he gets home from school and Jane returns from work. Jane uses the PHR to communicate with Junior’s sitter. The sitter can record information in the PHR at a laptop they keep in the kitchen. Each evening, when Jane gets home, she and the sitter review the day together and talk about how Junior is doing.
- Before Jane takes Junior for a doctor’s visit, she selects information out of the record that will give the care provider a representative view of how Junior has been doing in his daily activities.
