1.  DESCRIPTION OF SUBMISSION: (Example: 2 updated versions of hardbound texts (provided in alternate digital EPub, App and PDF formats), Bridge Year Implementation Kit containing CCSS Alignment Guide, additional manipulatives, performance task student workbook, teacher's guide, CCSS Student Intervention guide, 6 CCSS Teacher Training DVDs, 30 study guides) List ONLY those items which are part of the “Bridge Year” interim adoption.

2.  RESEARCH BASED: Include any research or information that demonstrates how the material has helped student attainment of the content. Attach addendum (maximum of two pages–one sheet front and back, font no smaller than 10 point.)

3.  QUALITY CRITERIA DOCUMENTATION: Concisely document how the submitted material meets the criteria. Give no more than 5 examples per criterion per category. Use no more than one page, one side, per criterion. Respond only to criteria items. Use font no smaller than 10 point).

4.  ALIGNMENT: Provide, at the time of review, correlations, pacing guides and other documentation that demonstrate alignment to the Common Core State Standards.

5.  ACCESSIBLE: NIMAS (National Instructional materials Accessibility Standard Agreement): Have assurance from the publisher agreeing to comply with the most current NIMAS specifications regarding accessible instructional materials.
