Ordering a Radiological Test with Anesthesia

1. Contact radiology via fax (353-8741) (or tube 32) with your radiology requisition form. No phone call is necessary. Indicate “ANESTHESIA REQUIRED” Radiology scheduling will coordinate with anesthesia.
Requisition forms must contain all of the following information.
o Patient Name
o Patient Medical Record Number
o Exam Description Requested
o Clinical Data/Diagnosis/reason for radiology procedure
o Printed and Signed name of requesting physician (legible)
o Pager or telephone number of requesting physician (legible)
o The attending physician’s name and pager number
o Special Instructions
o The time frame needed (today, this week, etc),
o Patient ambulation/transport status (i.e. cannot stand, wheelchair etc)
o Sedation needs (conscious sedation, anesthesia)
o If the patient is being discharged, please send the requisition as soon as possible and indicate the date of discharge.
o Consenting person contact info (if patient cannot give consent)
2. Radiology will page you back with the date and time of the scheduled procedure. This should occur within a few hours.

Holiday/Weekend/Overnight for Emergent/Urgent Procedures à
1. Contact radiology resident On-Call Pager 443-1749. Resident or Fellow will call anesthesia 353-1545 E-1 to add case and coordinate with technologist.
Tips & Reminders
1. E-listing of cases is not recommended unless it is truly urgent. If a case is e-listed by the scheduler, the case must be re-elisted the following day. Radiology will reschedule with anesthesia. If you need to contact the attending anesthesiologist managing the E list directly, page, 31581.
2. Patients undergoing anesthesia must be able to give consent, or if unable, consent has to be obtained by the next of kin, etc. In order to facilitate this and prevent delays, list the contact info for the consenting person on the requisition form.
3. The expected timing for completion of an anesthesia assisted radiological test is 24 hours. MRI tests may require 1-2 days from time of ordering until completion.
