Shepherd Mall Exam Site Schedule

ODMHSAS Training Center, 2401 NW 23rd Street, Oklahoma City, OK 73107

Please note all Exams times MUSTbe scheduled in advance by calling Ramona at 405-522-58366 or Ronda at 405-522-6852. Registration opens JUNE 28, 2010 and ends December 20, 2010. Walk-ins or alternate testers will not be accepted. Once you have scheduled a time with Ramona or Ronda, testers will be send a confirmation email that will include the exam instructions.

Testers are required to bring all required documents to exam site including:

  1. Email showing exam time confirmation.
  2. Exam Letter.
  3. Identification.

(No exceptions) Exam documents will not be sent to exam site.

Persons that are late will only be allotted remaining testing time for the exam. Please notify us of any cancellations at least twenty-four hours prior to testing.

Persons with special needs must notify us in advance with special needs requests. We will work to accommodate your testing needs, but test scheduling will adhere to indicated schedule time frame.
