GOVERNMENT REQUIREMENTS FOR THE ISSUANCE AF A(f) STUDENT VISA:

[To be submitted in four (4) copies: two (2) ORIGINAL and two (2) PHOTOCOPIES]

  1. Notice of Acceptance (NOA) from the school containing a clear impression of the school’s official dry seal, addressed to:

a. Student

b. The Director, VISA Division

Department of Foreign Affairs

  1. Five (5) copies of the 1998 Revised Original Personal History Statement (PHS), duly accomplished and signed by the applicant in English and in national alphabet, accompanied by personal seal, if any, original left and right hand prints on PHS and original photos;
  1. Transcript of records/ scholastic records duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence. ‘SEEN and NOTED’ STAMP is not acceptable;
  1. A notarized affidavit of support and proof of adequate and financial support (Bank Account Certification) to cover expenses of students accommodation and subsistence, school dues and other incidental expenses;
  1. Photocopy of passport pages where name, photo, birth date and birthplace appear; and
  1. Certified true copy of the Certificate of Eligibility for Admission (CEA) for students of Dentistry and Medicine.

In addition to the documents submitted to the DFA, the student shall submit the following requirements to the consular Officer at the Philippine Foreign Service Post upon notice of interview:

  1. ORIGINAL copy of the school’s Notice of Acceptance (NOA) containing a clear impression of the school’s dry seal, addressed to the student;
  1. ORIGINAL copy of the Certificate of Eligibility for Admission (CEA) issued by the CHED, if enrolled in courses or programs where restriction may exist due to shortage of facilities, as in Medicine or Dentistry;
  1. Police clearance issued by the national police authorities in the student’s country of origin or legal residence, authenticated by the Philippine Foreign Service Post having consular jurisdiction over the place; and
  1. Medical Health certificate issued by an authorized physician including standard size chest x-ray.

TIME FRAME OF SUBMISSION TO THE ADMISSIONS OFFICE OF THE ABOVE MENTIONED GOVERNMENT REQUIREMENTS:

FIRST SEMESTER – January to April 15

SECOND SEMESTER – June to September 15

SUMMER TERM – November 2 to January 31

N.B.: Applicant must be 18 yrs. old or above upon submission of the application

PRESCRIBED UNIVERSITY REQUIREMENTS:

  1. Letter of intent to enroll in UE;
  1. Transcript of Records (original and photocopy) duly authenticated by the Philippine Embassy in the Applicant’s country of origin or residence;
  1. Personal History Statement (PHS) (accomplished in CHED required form)
  1. Letters of recommendation (one from the Head Master and another from the former teacher or professor;
  1. Updated Alien Certificate of Registration (ACR) issued by the Bureau of Immigration;
  1. TOEFL (Test of English as a foreign language) result. (In the absence of TOEFL, the student shall be required to enroll in the UE Special English Class);
  1. Any other documents/requirements that the University may prescribed;
  1. Ten (10) 2’X2’ photos;
  1. The students are also required to pay:
  • Processing fee of seventy-five U.S. dollars ($75.00) or its peso equivalent at the prevailing exchange rate;
  • Acceptance fee of one-hundred and twenty-five U.S. dollars ($125.00) or its peso equivalent at the prevailing exchange rate

NOTE: The applicant’s admission to the University shall be dependent on his/her submission to the university of all prescribed requirements and compliance of the qualifying factor, i.e., he/she should have the CET-IQ (numerical value) score prescribed by the course he/she intends to pursue.
