New Rolling Log Instructions

This log has been expanded for several reasons. We have received a lot of feedback from DREs regarding the form and also have a need for specific data that is requested either by IACP for recertification or NHTSA for statistical purposes.

Most of the form is self-explanatory or the same as the old version, however several sections will be addressed.

  1. There is a new line for your email address at the top of the form.
  2. Reminder, the control number is now a three section; two digit-three digit-four digit number. It will reflect the year of the evaluation, the number that evaluation is for that year, and the total number of evaluations performed to date by the DRE. Ex 17-039-0973.
  3. Please use either a U, B, or R in the “Tox Type” section only. This will stand for Urine, Blood, or Refused as stated in the key at the bottom of the form.
  4. “Tox Results” will need to list the drug found NOT the drug category!! This is per NHTSA.
  5. Please use ONLY the letters indicated in the key on the bottom of the form for the “Role” and “Type.” Please note that a certified DRE should only be using the “E” for evaluator or “I” for instructor under the “Role” section. The other letters are for use by DRE candidates. Please note the only types of evaluations are enforcement “E”, instructor “I”, training “T”, and other “O”.
  6. There is a new line at the bottom of the form for the information on the one evaluation that should be witnessed by an instructor. Please provide the name and DRE number of the instructor as well as the evaluation number that instructor witnessed.
  7. The miscellaneous section has been expanded to allow for more notes. Please include a note in this column regarding the evaluation witnessed by the instructor listed at the bottom of the form. If more than one evaluation was witnessed or an evaluation was witnessed by a different instructor, the miscellaneous column is where that should be noted. (See example rolling log)
  8. You do NOT need to redo your entire rolling log to conform with the new form. Please begin using the new log immediately and note that any evaluations performed after October 15, 2017, will be required to be on the new form.
  9. The form is called a rolling log for a reason. A new form should NOT be started with each new year. Just continue adding evaluations and only start a new form when you run out of room. Also, your entire log is required for recertification!
