Miss Frazee Outstanding Teen Contestant Agreement

I ______am between the ages of 13 and 17. I will not turn 18 years of age on or before September 1, 2014. I am or will be 13 by date of pageant. I am a student of Frazee-Vergas School District. I fully understand that if I receive the title of Miss Frazee Outstanding Teen, I will be obligated to fulfill the duties of my title:

1. I will act in the capacity of ambassador for the Frazee Area Community for one full year, starting the night I receive the title, until my successor is crowned the following year.
2. I will make every reasonable attempt to attend all appearances scheduled for me. Appearances include but not limited to, Perham, Detroit Lakes, Wolf Lake, Vergas, Oktoberfest and Turkey Days during year of reign.
3. Contestant will sign contract for Miss America/Miss Minnesota Program. Copy available upon request.
4. I will dress in appropriate attire when making appearances as Miss Frazee Outstanding Teen.
5. I will not make any appearances as Miss Frazee Outstanding Teen without prior approval if the Miss Frazee Scholarship Committee.
6. I will conduct myself in a manner that will command respect from the public.
7. All my actions will reflect high moral standards.
8. I am not nor ever have co-habituated with a member of the opposite sex. I must and will not co-habitat during my reign as Miss Frazee Outstanding Teen.
9. I have never been married, divorced, nor had a marriage annulled.
10. I have never been and presently not pregnant.
11. I have never been convicted of any crime and do not have criminal charges pending against me.
12. Any fund raising projects need to be approved by pageant committee.

I fully understand that if I default on any of the above listed rules I will be obligated to immediately relinquish my title as Miss Frazee Outstanding Teen.

The Miss Frazee Scholarship Pageant Committee will have ultimate authority in the decision making process of determining the behavior and actions of Miss Frazee Outstanding Teen. The decision of the committee will be final.

I understand that I will forfeit all scholarships if my title is terminated. It will be my responsibility to reimburse the Miss Frazee Scholarship Pageant Committee for all expenses that were paid on my behalf; including but not limited to: tiara, entry fee to the Miss Minnesota Pageant, and the pageant preparation.

SCHOLARSHIP AWARDS REDEMPTION PROCESS:

I understand that all scholarship money from The Frazee Outstanding Teen Pageant Program will be paid according to the following schedule:

1. Completion of Scholarship Award Form (Available at The Yak Shack)
2. Submission of official transcript proving good standing, in accordance with Federal Financial Aid.
3. Statement of fees due.

Frazee Outstanding Teen Contestants and can have funds paid in their behalf to any of the following options.

• Professional lessons
• Post-secondary school of her choice

Miss Frazee Outstanding Teen will be eligible to receive her full scholarship upon completion of her reign. Above guidelines also apply.

The Miss Frazee Scholarship Pageant Committee will have the ultimate authority in distributing scholarships.
