Each member of the Corporation who wishes to be considered for endorsement to serve on the Board of Directors must provide on this form the information requested herein. Conciseness is encouraged.

Candidates should be aware that donor advisory organizations recommend highly that members of the governing body of a non-profit entity provide some financial support to it.

The content of your responses will be used only by the Nominating Committee and the incumbent Directors who are not standing for re-election to determine the candidates who will be endorsed for the upcoming election.
