In consideration of my/our receiving the dog described above THE UNDERSIGNED agrees to abide by the "Terms of Adoption" listed below:

The undersigned adopter(s) understands that he/she must abide by the terms of the agreement. The undersigned adopters(s) each, separately or jointly, does hereby release and hold harmless Metrolina Dogs Hope Rescue and its rescue volunteer(s) from any claims, damages or actions incurred as a result of this adoption or caused by any action of the dog transferred herein.

Return: The adopted dog may be returned to Metrolina Dogs Hope Rescue (or representative/agent) within two weeks of the date of adoption for full refund of donation if the adopter is not satisfied with any aspect of the adoption. After two weeks from the date of adoption no refund will be given if the dog is returned to MDHR (or associate of MDHR).

Transfer of Ownership: The adopted dog may not be transferred to any other person for any reason. If the undersigned adopter is forced to relinquish custody of the dog at any time, said dog will be given only to MDHR.

Refund: There will be no refund or reimbursement for any expenses incurred by the adopter for the adopted dog, even if it is returned to MDHR.

Reason for adoption:

* The adopted dog is to be kept as a house pet/ companion, not as a gift, not as a working animal or guard dog.

* It shall not be used for medical or any other experimental purpose.

* It shall not be used for breeding.

Maintenance of Dog:

The adopter of the dog agrees to the following:

* The adopted dog will be licensed in accordance with the laws in the jurisdiction in which the dog resides, including laws governing the immunization of dogs against Rabies.

* The adopted dog must wear an identification tag at all times.

* The adopted dog will be cared for humanely, including good quality food, shelter and water available 24 hours a day.

* THE DOG IS TO BE EXERCISED IN A FENCED YARD OR ON A LEASH AT ALL TIMES AND NOT ALLOWED TO BE FREE TO ROAM.

* I give permission to MDHR to visit my premises at any time to insure this agreement has been kept.

Health Program:

Metrolina Dogs Hope Rescue (MDHR) agrees to the following:

* To have all dogs altered (spayed or neutered) before transfer of ownership.

* To provide for dogs the following initial inoculations, Distemper, Hepatitis, Leptospirosis, Parainfluenza, Parvovirus and Rabies.

* To have all dogs checked for internal and external parasites and treated if necessary.

* To have maintain all dogs on monthly heartworm preventative.

Adopter agrees to the following:

* The dog is to be seen by a veterinarian yearly, given all necessary vaccines;

* checked for heartworm each spring; maintained on heartworm preventative year round;

* stool sample checked for worms as necessary and seen by the vet when ill.

Loss: If the adopted dog is lost or stolen, the adopter will notify MDHR immediately, at least within (2) days, so MDHR can help locate the adopted dog.

Reservation of Rights: MDHR (and its agents) reserves the right to enforce this contract in order to protect the welfare of the adopted dog. It is understood that representative from MDHR may examine and may make inquiry about said dog at any time. If the terms and conditions of the adoption Agreement are not upheld, MDHR reserves the right to terminate the Agreement and to reclaim the dog.

The adopter must agree to the following:

* I hereby acknowledge receipt from MDHR of the dog described above.

* I understand that MDHR makes every attempt to carefully evaluate the health and temperament of dogs in its care, and to only place dogs of sound temperament. However, I accept the dog as is and assume all risk and financial responsibilities of ownership.

* I hereby release MDHR (and its agents or associates) from any liability associated with any illness of the dog or damage; accident or injury hereafter caused by said dog.

* I agree that MDHR will not in any way be held responsible for the conduct of this dog after its adoption.

* I understand that some or all of the information provided on the Adoption Agreement regarding said dog could have been received by MDHR from a third party; I understand that MDHR does not warrant the accuracy of such information.

* I understand that any KNOWN problems have been disclosed.

* In addition, if the dog was found wandering or lost, I acknowledge that MDHR has no knowledge of the prior owner(s) of the dog, nor the conditions under which the dog was living.

* If at any time I desire to relinquish custody, or MDHR demands its release for failure to comply with agreement, I agree to return said dog to MDHR, making no charges of any nature for licensing, care, food or other services or items.

* I agree to pay any reasonable attorney fees and court expenses if MDHR must bring legal action against me to enforce this Adoption Agreement.

* I understand that any sum of money I have given to MDHR is to be used for its work in caring for this and other dogs and is not refundable except as provided in "Terms of Adoption."

* Adoption donation: The minimum donation for adopting a purebred dog from Metrolina Dogs Hope is $250. The donation for young puppies and dogs with cropped ears may be higher. The donation for adopting a mixed breed dog will be lower but not less than $150. All donations are used to offset veterinary and maintenance/housing expenses for our foster dogs.

Return Check Charge: There will be a $25.00 charge on all returned checks submitted by the adopter.

Metrolina Dogs Hope Adoption Contract – agreement

I hereby acknowledge that I have read and understood this Adoption Agreement and agree to abide by all of its rules and regulations set forth herein. I pledge that I will keep the dog described herein as a part of my family.

I HEREBY VERIFY THAT I AM EIGHTEEN (I8) YEARS OR OLDER; THAT I FULLY UNDERSTAND MY OBLIGATIONS AS DESCRIBED ABOVE AND THAT FAILURE TO COMPLY WITH THESE OBLIGATIONS WILL BE A BREACH OF CONTRACT WHICH GIVES MDHR THE LEGAL RIGHT TO TERMINATE ANY OWNERSHIP OF THE ANIMAL THEREBY THE LEGAL RIGHT TO RECLAIM THE ANIMAL WITHOUT REFUND.
