GENERAL APPLICATION FOR 2005 SCHOLARSHIPS OFFERED TO

STUDENTS THAT LIVE IN MERRICK COUNTY OR

ATTEND A MERRICK COUNTY SCHOOL

Before you begin, please read the instructions below carefully. Failure to follow these instructions may result in your application being disqualified.

This scholarship program is available to residents of Merrick County school districts. One scholarship application form covers all scholarships available to high school seniors due in the Foundation office, or postmarked, on February 17, 2005.

To ensure that your application is completed and submitted correctly, please adhere to the following requirements:

1.  To view the overall format of the document, print out a blank copy of the form. Pages 1-7 include instructions and detailed information about scholarship selection criteria. Pages 8-21 comprise the actual application form. Note that there are instructions within the form itself.

2.  Use the “Tab” key to move from field to field. To avoid altering the form, DO NOT use the “Enter” key!

3.  Periodically SAVE your information!

4.  Proof your work! It may be helpful to have your parent or guardian review the information.

5.  Review the Application Checklist.

7. Save your final Scholarship Application Form to a 3 ½” Floppy Disk (Available from your Guidance Counselor) Name the file: Your Name, MF Senior App 2005

8.  Print the Scholarship Application Form. To avoid printing instructions and scholarship application guidelines, start printing at page #8.

9.  Student must sign the Application form.

10.  Student and parent/guardian must sign the Financial Need Information Form.

11.  Submit your saved application form on a 3 ½" Floppy Disk and submit one printed copy of your application to the Foundation on or before 4:45 p.m. February 17, 2005. Applications received after this date will not be accepted. Note: Please label your 3 ½" Floppy Disk with your name and address.

SCHOLARSHIP PROGRAM

2005-2006 ACADEMIC YEAR

QUALIFICATIONS & GUIDELINES

Please check all the scholarships you wish to apply for that fit your course of study and your particular community. To be considered for one of these scholarships, the following regulations must be met:

1)  All scholarship recipients must be enrolled in an accredited college, university, technical or vocational school that qualifies as a tax-exempt institution.

2)  All scholarships shall be for the purpose of tuition and fees required for enrollment or attendance at an educational institution or for books, supplies and equipment required for courses of instruction, under the policies of the school or educational institution. Room and board is not covered.

3)  The funding of all scholarships will be made directly to the school of higher education and designated for the use of the specific student selected, upon verification of enrollment of the student.

4)  If the scholarship recipient drops out before the end of the school term or for any reason does not use the full amount of this scholarship in the academic year for which it is awarded, the unused portion shall be returned to the Merrick Foundation, Inc. and placed in the appropriate scholarship fund.

5)  All applicants are required to supply the Merrick Foundation, Inc. with a minimum of 2 confidential signed letters of recommendation. Some scholarships require specific people and/or more than 2 recommendations, see specific scholarship requirements. Letters can be used for multiple scholarships. Recommendation forms are available to use and can be completed and printed from our website (www.merrick-foundation.org) or obtained at our office. You may turn them in with your application or have your references send them to: Merrick Foundation, Inc., P O Box 206, Central City NE 68826

a)  Person qualified to furnish a character reference (i.e. involvement in church, community, work or job related, etc.).

b)  Educator who is familiar with your scholastic and leadership abilities as well as your achievements at the high school level.

6)  All applicants are required to supply the Merrick Foundation, Inc. with an official transcript of work completed through your first semester senior high school level. (Must include school verification of your class rank and GPA)

7)  Financial Need Information Form - Refer to the scholarship requirements to determine which scholarships are based on financial need. They are marked with an *.

8)  Applications, including all attachments, must be postmarked no later than February 17, 2005, or if hand delivered, no later than 4:45 p.m. on February 17, 2005.

SCHOLARSHIP REQUIREMENTS - HIGH SCHOOL SENIORS

MERRICK FOUNDATION GENERAL SCHOLARSHIPS*

¨  Upper 1/2 of graduating class

¨  Merrick County resident or attending a Merrick County school

¨  Students enrolled in a 4-year course: two scholarships for students attending Palmer High School, two scholarships for students with Clarks addresses(preference), two scholarships for students attending Nebraska Christian High School, and two scholarships for students with Silver Creek addresses(preference), two scholarships for students with a Western Merrick County address(preference).

¨  Students enrolled in a 2-year course: one scholarship for a student attending Palmer High School, one scholarship for a student with a Clarks address(preference), one scholarship for a student attending Nebraska Christian high school, one scholarship for a student with a Silver Creek address(preference), and one scholarship for a student with a Western Merrick County address(preference).

¨  Four scholarships for students enrolled in a 4-year course attending Central City High School

¨  Two scholarships for students enrolled in a 2-year course attending Central City High School

¨  Based on (1) Good citizenship in school and community (2) Acceptable scholastic achievement (3) Evidence of perseverance toward goals (4) Financial need

¨  An essay (no longer than one single-spaced page) explaining your educational goals.

INDUSTRIOUS STUDENT SCHOLARSHIPS*

¨  2.0 - 3.5 GPA or C to B+ average

¨  Any form of continuing education

¨  Paragraph stating why you are continuing your education, why this scholarship will benefit you, and why you have a financial need

¨  Resident of Merrick County

LEONARD & DORIS AERNI MEMORIAL SCHOLARSHIP

¨  Merrick County resident or attending a Merrick County school

¨  Handicapped graduating senior

¨  Essay of approx. 300 words explaining about handicap

¨  Pursuing a degree at a community college, technical school, or business school

¨  One renewable and one nonrenewable scholarship given

¨  Must reapply each year for renewable

KAREN & DANNIELLE BANNISTER MEMORIAL SCHOLARSHIP

¨  Graduating senior at Aurora, Central City, or High Plains Community high schools.

¨  Pursuing English major or minor - 4 year degree

¨  Letter of recommendation from English teacher

¨  GPA average or above

CHARLES BENNER MEMORIAL SCHOLARSHIP

¨  Central City high school senior with GPA of average or above

¨  Must have participated in FFA during high school

ERVIN & BONNIE BLAUHORN SCHOLARSHIPS*

¨  Three are given annually to high school seniors in Central City or Palmer with at least one going to a Palmer senior

¨  Upper 1/4 of graduating class

¨  Based on character, personality, general health, and financial need

¨  Applicant must be enrolled in a 4-year course

CENTRAL CITY CARE CENTER SCHOLARSHIP

¨  Resident of Merrick County

¨  Senior at Central City, High Plains, or Nebraska Christian High Schools

¨  Pursuing a career in the medical profession

MYRTLE AND JIM CLARKE SCHOLARSHIP*

¨  Based on academic grades or overall achievement outside school

¨  Otherwise lack the resources, including student loans, to continue education beyond high school

¨  Attending Central City or Nebraska Christian High schools

OR

¨  Resident of Clarks, Fullerton, Hordville, Polk, or lake areas south Central City

MARSHALL SUCK-COLLINS MEMORIAL SCHOLARSHIP

¨  Palmer High School senior

¨  Upper 1/2 of class

¨  Active in music program at Palmer High School

¨  4 year course of study (first two may be in Community College)

JOEL HOLBROOK MEMORIAL SCHOLARSHIP

¨  Graduating senior at Central City high school

¨  Participated in Band all four years of high school

BONNIE KOHL MEMORIAL SCHOLARSHIP

¨  Resident of Merrick County

¨  High School senior with a 3.5 GPA or higher and enrolling in Bachelor of Science in Nursing program, endorsements from one school official, one teacher, and any one of the following: minister, priest, scout leader, employer, coach, or other youth leader who has a personal knowledge of the applicant

¨  May be continued at the Board's discretion if applicant is continuing study for Nurses degree and maintains 3.0 GPA.

ROBERT & LILLIAN KRUG MEMORIAL SCHOLARSHIP

¨  High School senior with Merrick County address

¨  GPA average or above

¨  Active in Merrick County 4-H program for at least 4 years and/or an active member of a Merrick County FFA Chapter for at least 2 years

¨  Three letters of recommendation from teachers, pastors, or employers.

¨  Essay on how 4-H and/or FFA has benefited you to prepare for your future and why you are furthering your education.

CHARLES & ANN KUHLMAN MEMORIAL SCHOLARSHIP
