NOTE: This application MUST be returned with a $300.00 deposit (non-refundable). If a deposit is not received your team could forfeit their position in the League. Competitive and Recreational Divisions.

Season begins September 13, 2016** Teams will play a double header.

Teams pay $15.00 for Referees before each game

Fee $1,504.42 + HST ($195.58) = $1,700.00

SAVE $300.00 off the league fees – If your team pays the entire league fees in full on or before the 1st game. The fee would be $1,400.00

$25.00 OSA & District Registration fee for each player. **SEE NOTE ON BACK**

There is a $40.00 fee for any returned cheques.

v  No team will be allowed to play their 5th game unless all fees are paid in full, this includes the player registration. All games will be forfeited until this is paid.

v  2 women MUST be on the field at all times for the Co-Ed games, if 2 women are not available the team will have to play shorthanded. A minimum of 1 women on the field is required to start the game.

v  Top 4 teams will make the playoffs

v  Games will be 2 x 20 minutes

v  10 games guaranteed (League & Playoff games)

v  Any teams registering after the 1st session will automatically be put on a waiting list.

v  All players MUST register before your 1st game. Any additional players MUST register before they play and no new registrations will be accepted after the 7th game.

v  All players will be required to take a picture and fill out the required form with the league staff before they will be permitted to play.

v  All teams MUST wear numbered shirts.

v  Winners of the League will receive T-Shirts and Playoffs winners will receive medals at the end of each session.

v  Tuesdays, Wednesdays & Thursdays will be on a rotating schedule with the 1st game beginning at 9:00pm (If earlier times are available, game time will change)

v  Sunday’s 1st game time is 3:00pm. (If earlier times are available, game time will change)

v  All teams will play at least one double header. All playoffs are double headers.

v  Teams may be refused based on previous discipline and payment issues

v  FIGHTING WILL NOT BE TOLERATED. Any individual caught fighting will automatically be expelled from the League and any other Leagues that he may play in at this facility.
