Memo Regarding Changes in Transition Planning for Youth

Changes to Transition to Adulthood Planning Process for Youth

Effective August 1st, 2017, the following changes will take effect for the Transition to Adulthood Planning Process for Youth.

·  All providers of, “Children’s” Targeted Case Management (TCM) and “Children’s” Behavioral Health Homes (BHH) will no longer need to enter Youth Transition to Adulthood information into the Departments Enterprise Information System (EIS).

·  If the youth has reached the age of 17 and a referral to OADS is indicated, please provide the most current Psychological Evaluation that includes a full scale IQ and adaptive scores; an OADS Release of Information signed by the Guardian to an OADS intake worker in your district (See Intake contacts below)

·  Transition to Adulthood District Transition Teams will still be tracking youth 16 and older and they can be available to you for assistance (contact information attached)

·  Judy DeMerchant will continue to reach out you as youth turn 16 years old.
