Medical Examinations & Immunizations for all St. Theresa Students

  1. Students entering preschool, kindergarten, 6th grade, and students new to Illinois schools must have a physical examination. The Illinois Certificate of Child Health Examination done within the year prior to the school start must include:

•The health care provider’s signature and date of exam.

•The student’s health history (on back side) completed and signed by parent/guardian.

•Height, weight, BMI, & B/P.

•Screening for diabetes. Positive results require medical follow-up.

•Screening for lead exposure only for students under age six. Positive results require additional lead level testing.

  1. Students entering kindergarten, second, and sixth grade are required to have a dental examination.

3. Students entering kindergarten and new to Illinois schools are required to have an eye examination.

REQUIRED IMMUNIZATIONS FOR ILLINOIS STUDENTS

Diphtheria, Tetanus, Pertussis (DTP/DTaP)

•Have received 3 or more doses of DTP/DTaP at the recommended intervals with the last dose received on or after the fourth birthday.

Pre-Kindergarten and Kindergarten:

•Have received 4 doses of DTP/DTaP at the recommended intervals with the last dose received on or after the fourth birthday. Grades 6, 7, and 8:

•Have received 1 booster Tdap.

Polio (IPV)

•Have received 3 or more doses of IPV at the recommended intervals with the last dose received on or after the fourth birthday.

Pre-Kindergarten:

•Have received 3 doses of IPV at the recommended intervals.

Kindergarten :

•Have received 4 doses of IPV at the recommended intervals with the last dose received on or after the fourth birthday.

Hepatitis B (HBV)

Pre-Kindergarten and Grades 6, 7, and 8

•Have received 3 doses of Hepatitis B vaccine at the recommended intervals.

Varicella (Chicken Pox)

Pre-Kindergarten:

•Have received 1 dose of varicella vaccine after the first birthday.

Kindergarten and Grades 1, 2, 6, 7, and 8

•Have received 2 doses of varicella vaccine with the first dose on or after the first birthday.

Measles (Rubeola) German Measles (Rubella, 3 Day) Mumps – (MMR)

Pre-Kindergarten:

•Have received 1 dose of MMR vaccine after the first birthday.

Kindergarten through Grade 8 :

•Have received 2 doses of MMR vaccine with the first dose on or after the first birthday.

Meningococcal conjugate (MCV4)

Grades 6 and 7:

•Have received 1 dose of meningococcal conjugate vaccine (MCV4) on or after 11 years of age.

Additional immunizations for students less than 5 years old (Pre-Kindergarten)

Haemophilusinfluenzae type b (Hib)—Have received the series of Hib vaccine.

•Pneumococcal—Have received the pneumococcal vaccine according to the PCV schedule.

According to district policy and Illinois School Code, students out of compliance with health requirements will be excluded from school. Information on vaccines may be found at
