POLICY: DISEASE REPORTING

SUMMARY: To notify the Iowa Department of Public Health of outbreaks of any kind, unusual syndromes, or uncommon diseases. These could be infectious, environmental or occupational in origin. It also includes any kind of biological, chemical or radiological agent or toxin. The attached list includes the reportable diseases/conditions (but are not limited to).

DESCRIPTION:

1.  Sexually Transmitted Diseases (STD):

A.  All patients with positive STD results will be reported verbally to the Iowa Department of Public Health’s Regional Disease Prevention Specialist or Laboratory Director, or by filling out the state reporting form.

B.  Confirmation of STDs:

1)  Physical exam and patient history.

2)  Confirmation by laboratory testing:

a. RPR & FTA for Syphilis.

b. Vaginal, penile secretions and/or urine specimen for Gonorrhea and Chlamydia.

C.  Procedure:

1)  The Provider will give the positive laboratory results to their nurse.

2)  If a patient has a positive RPR or FTA, an appointment will be scheduled for the patient so the provider can get a more in-depth history of possible previous treatment. Treatment will be instituted. Clinical staff assisting the provider will forward results to assigned staff responsible for reporting and initiating appropriate forms.

3)  If test results are positive for Gonorrhea and/or Chlamydia, the patient’s Provider will initiate treatment (medication provided by the Iowa Department of Public Health) and appropriate follow up appointments will be scheduled. The Provider’s nurse will forward results to the case manager for state reporting.

4)  HIV testing and treatment for partners will be recommended as indicated by the SCHC Provider.

2.  Hepatitis Reporting:

A.  A diagnosis of confirmed Hepatitis results will be verbally reported to the Iowa Department of Public Health’s Regional Disease Prevention Specialist or Laboratory Director.

B.  Confirmation of Hepatitis:

1)  Physical exam and patient history.

2)  Confirmation by laboratory testing.

C.  Procedure:

1)  Initial reporting of positive testing performed by an outside laboratory is responsible for notifying the state if any positive results.

2)  SCHC Providers will give all positive laboratory results to their nurse who will follow-up and complete the state reporting.

3.  All Other Reporting:

A.  Follow the attached list of Reportable Diseases and the reporting requirements.
