We have considered your request and conducted an appropriately thorough search within the above formats. In accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) we can confirm that the practice does not currently hold any emails, letters, correspondence or records of telephone conversations containing information about yourself.
In line with the Information Commissioner’s directive on the disclosure of information under the Freedom of Information Act 2000 your request will form part of our disclosure log. Therefore, a version of our response which will protect your anonymity will be posted on our website.
We also have no documented discussions by X about yourself with X, so we are therefore surprised at your direct quotation of X without X knowledge.
