1. Brief Description of the Acute Services Division of NHS Greater Glasgow and Clyde
The Acute Division of NHS Greater Glasgow and Clyde is the largest group of adult acute hospitals in Scotland. We provide a wide range of services from community-based care through to the full range of general hospital services.
There is a significant amount of complex surgical work carried out, as we are home to a number of the country’s tertiary and national services. Glasgow is also home to the world famous Beatson oncology centre.
We enjoy close links with Glasgow’s three universities and make a significant contribution to teaching at both undergraduate and postgraduate level. Research also has a high profile within the organisation. Excellent facilities for students and their tutors are provided in the brand new, £15 million, state of the art, WolfsonMedicalSchool, based at GlasgowUniversity.
Employing more than 14,300 staff, we serve a core catchment population of 560,000. Our patients not only come from Greater Glasgow and Clyde but also, in some instances, from the whole of the West of Scotland and for our national services, from the whole of Scotland.
We have seven hospitals north of the River Clyde, all with teaching responsibilities: Glasgow Royal Infirmary (including Princess Royal Maternity Hospital), StobhillHospital, Gartnavel GeneralHospital, GlasgowDentalHospital and School, DrumchapelHospital, BlawarthillHospital and LightburnHospital. South of the river are the QueenElizabethUniversityHospital and the Victoria Infirmary. The RoyalAlexandraHospital is in Paisley, the InverclydeRoyalHospital is in Greenock and the Vale of Leven District General Hospital is situated in Alexandria.
2 Hospital and Clinics Where Services are to be provided
Glasgow Royal Infirmary is a University teaching hospital located in the east end of Glasgow. The orthopaedic department serves a local catchment population of approximately 340,000, and provides tertiary service in the subspecialties.
There are currently 16 Substantive Consultant Orthopaedic Surgeons in the Unit where the full range of trauma and elective orthopaedic sub-specialties is covered.
The orthopaedic wards at Glasgow Royal Infirmary are located in QueenElizabethBuilding in close proximity to the theatres and ITU and also in the Surgical Block. Inpatient facilities for both trauma and elective work are based here.
There are three laminar flow theatres for Elective Orthopaedic work. There are two trauma theatres at Glasgow Royal Infirmary, one general trauma theatre and one sub-specialist trauma theatre. Both are located in the main theatre suite, with one available for the management of Orthopaedic and soft tissue trauma seven days a week and the other four days a week. Day surgery sessions for Orthopaedics are available at the Ambulatory Care Hospital (ACH) on the Stobhill site where 23 hour stay facilities are also provided..
Elective Orthopaedic and Fracture clinics are based at Glasgow Royal Infirmary with a dedicated X-ray facility and medical records department. The processing of patients through the Outpatient department is greatly facilitated by extended role Specialist Orthopaedic Physiotherapists and Podiatrists who initially assess a high proportion of referrals from Primary care. A total of 38 sessions per week of Specialist Physiotherapist time is allocated to this task.
The highly successful fracture clinic redesign has resulted in a significant reduction in unnecessary attendances for simple fractures. Guidelines and protocols are agreed with the Emergency Department for many of the fractures and patients are only seen following a triage of their x-rays and Emergency Department assessment by consultants in a virtual clinic.
A number of post-operative follow-up clinics are run by nurses, physiotherapists and podiatrists. Currently all primary and revision arthroplasty, soft tissue knee and arthroscopy patients are followed a dedicated Outcome Assessment Service with uniform data collected onto a database.
Glasgow Royal Infirmary is a major teaching hospital and has all major specialties on site with the exception of Neurosurgery and Cardiothoracic surgery.
There is an excellent Intensive Care Unit run by the Anaesthetic Department at Glasgow Royal Infirmary.
The Centre for Rheumatic Diseases is a professorial unit based at Glasgow Royal Infirmary and enjoys a close relationship with the Orthopaedic Department. The regional oncology service for the West of Scotland at the new Beatson Centre is located on the Gartnavel site. As a result, the department has the largest workload dealing with bony metastastic disease of any orthopaedic unit in the country. There are also close ties with the Imaging Department with ready access to both CT and MRI scanning, including a dedicated extremity scanner.
The department also works with the University of Strathclyde Department of Bio-Engineering and the National Centre for Prosthetics and Orthotics.
The Consultant members of the department are involved in Undergraduate teaching for the University of Glasgow. The Unit is also recognised for Basic Surgical Training and for Higher Surgical Training in Trauma and Orthopaedics. Currently there are 9 ST 3 and above trainees and 6 at the more junior level. There is a postgraduate teaching programme for higher surgical trainees organised on a West of Scotland basis. Education is strongly supported within the unit with daily multi-disciplinary trauma meetings, regular educational meetings and a monthly audit meeting.
