• Second semester: July 6-10, 13-17, 20-23, 7:30 a.m.-12 noon

COSTS

• Tuition: $166 per semester

• Bus fee: $178 per semester

TO ENROLL

1.Complete the registration form on the next page. If you're enrolling for both semesters, indicate both course selections on one registration form.

2.All students must be residents of Lake Park High School District 108. The residency requirement will be based on the first day of the summer school term in which the student is enrolled.

3. Have your counselor approve all course selections before you pay tuition or bus fee.

4. Have your parent/guardian sign your registration form before you paytuition or bus fee.

5. Submit your form and pay all tuition or bus fees in the main office at either campus beginning Monday, April 6. Make checks payable to LakeParkHigh School.

IMPORTANT NOTES

1. Summer school is a self-supported program based on enrollment.

2. Class size is limited. FIRST SEMESTER REGISTRATION CLOSES FRIDAY, MAY 22. Second semester registration closes Friday, June 19.

3. Courses will run depending on enrollment. If your first or second course choice is not available, you may be offered to earn the credit throughFuel Education (on-line course on back page). If all three options are not available, you will receive a full refund.

4. First semester refunds will not be given after June 12. Second semester refunds will not be given after July 10.

5. On the day of a student absence, a parent or guardian is required to notify school officials of the absence before 10 a.m. Please call 630-295-5222to report an absence.

6.ATTENDANCE POLICY: Because attendance is so important at summer school, the following regulations will be enforced. A student will be removed from summer school without a refund if the student is absent for 3 days regardless of the reason. An absence is defined as missing one hour or more of the class. The absence will be recorded if the student arrives after 8:30 a.m. or leaves school before 11 a.m. A tardy is defined as being absent from the class for less than one hour. A tardy will be treated as a one-half day absence. Any combination of absences and tardies which totals 3 days will result in the student being removed from summer school.

7. Students removed from summer school will lose tuition and transportation fees.

8. Driver Education Classes: Students should contact the main office at East Campus for information concerning Driver Education—630-295-5221.

9.Second semester bus will run depending on enrollment.

Please Note

The FOLLOWING Classes are dependent on enrollment. Students may be placed in A Fuel Education course if the class they sign-up for is cancelled due to low enrollment.

All summer school classes will be offered for credit recovery only, with the exception of Consumer Education.

Students will not earn a grade replacement if they take a lower level course. For example: Geometry students taking Geometry B would not earn a grade replacement, only credit.

Students requesting Driver Education should contact the main office at East Campus.

Algebra II students are required to have a graphing calculator.

A student that signs up for Summer School must read and agree to follow all rules provided on the next page.

1. No student is allowed to leave the building without permission from the Summer School Director.

2. Students arriving late or leaving early must check in or out of the main office.

3. There will be no cafeteria food service during summer school.

4.Use or possession of smoking materials, chewing tobacco, and/or snuff is not permitted on school grounds (building, property, parking lot, bus) and will result in the student being removed from summer school for the remainder of the semester. Cutting class or leaving the building without permission will cause the student to be removed from summer school for the remainder of the semester. Any gross disobedience or misconduct (see below) will cause the student to be removed from the entire summer school program. A student referred for fighting or for two other incidents may be removed. Refunds will not be given to a student removed from summer school for any reason. If removed during the second semester, first semester summer school credit will be retained.

5. Resource center or computer lab disruptions will result in loss of privileges of those areas. A second incident could result in being removed from the summer school program.

6. Students waiting for later classes must remain by the main entrance.

7. Parking: Students who drive may use the parking lot.

8. A student will be removed if he/she accumulates three full-day absences in a semester. A tardy (arriving between 7:30 a.m. and 8:30 a.m.) equals a one-half day absence. A student arriving after 8:30 a.m. or leaving before 11 a.m. is considered absent for the entire day. Any combination of tardies/absences totaling three days will result in the student being removed from summer school without a refund.

9. Students may not use hallway lockers. Locks will be removed and contents of the locker will be confiscated.

10.Inappropriate behavior on a school bus may result in a suspension of bus transportation services and/or the student being removed from summer school (see #4 above). No refund will be given.

11.Cheating in any form will cause the offending student to lose credit for the assignment and the parent will be contacted. A second instance of cheating will cause the student to be withdrawn from the course with no credit of refund.

12.When students arrive in the morning, they must go directly to their classrooms.

13. Students are required to attend the last day of the semester. Early releases willnot be allowed.

DISCIPLINE

Two principles govern all rules at Lake Park:

•conduct which is disruptive of the educational process is prohibited.

• conduct which infringes upon the rights of others is prohibited. All students must be knowledgeable of the school rules which relate to conduct and discipline.

GROSS DISOBEDIENCE OR MISCONDUCT

A student guilty of a gross disobedience or misconduct will be removed from summer school. An action of gross misconduct may also be brought to the attention of the police for criminal prosecution. A student, while on school property, on school buses, or at any school-sponsored activity, shall not:

• possess, consume, or distribute any controlled substance (alcohol, narcotics, marijuana, etc.) or look-alike drug.

• intimidate, threaten, bribe, strike, or physically assault (which includes gang involvement) a school employee, a member of the Board of Education, a bus driver, a student, or a school visitor.

• damage or vandalize property of the school District/host District or the property of a school employee, a member of the Board of Education, a bus driver, a student, a school visitor, or the company with whom the District contracts for transportation service.

• steal and/or possess stolen property of the school/host school or a school employee, a member of the Board of Education, a bus driver, a student, a school visitor, or the company with whom the District contracts for transportation services.

•activate a school fire alarm without appropriate cause.

• make a bomb threat.

•set a fire within a school building; to other school property; to the property of the company with whom the District contracts for transportation services; or to the property of a school employee, a member of the Board of Education, a bus driver, a student, or a school visitor.

• possess or set off fireworks or other explosive substances.

• possess or cause to be brought to school or to any type of school-sponsored activity firearms, knives, lethal weapons, or disabling devices (mace, pepper gas, etc.) of any type; potentially dangerous objects or substances; or look-alike weapons. (Illinois Gun Free Schools Act 1994) (Chapter 38 of the Illinois Criminal Code, Section 24)

BUSES

The Summer School Program provides bus service to and from school for those students who pay a fee.

Students must:

• obey the bus driver.

• remain seated while the bus is moving.

• keep all material inside the bus. Students and parent(s) or guardian(s) are held responsible for any damages.

• keep head, arms, legs, hands and feet inside the bus (windows must not be lowered below the mark indicated on the bus).

A student who becomes a serious or chronic disciplinary problem will have bus privileges revoked for the remainder of summer school with the loss of transportation fees.

Fuel Education is an internet-based computer software program that allows students to make up credits for a variety of classes. Students are required to complete the work in class, which parallels the classes offered during the regular school year. The classes will be held in the East Campus computer lab. Each student will receive a folder with a syllabus and a course plan indicating their designated amount of lessons they need to complete each week in order to finish by the end of the semester. Once a student has completed his/her course(s), the student is dismissed from the program. The instructor will provide individual help during designated class sessions, check student progress on a weekly basis, and determine final course grades, which are on a pass/fail basis.A grade replacement will not beoffered, only course credit. It is expected that all students will complete their course by the final day of class.
