Briefly describe your organization’s mission and history, including previous experience in providing arts and culture projects or events in the community you serve.

a. Describe the project and provide specific details what your organization plans to do, how it will be accomplished, and where and when the program take place, including a detailed timeline. Note: Projects must be completed between May 1, 2017, and April 27, 2018.
b. How does the proposed project advance specific goals and/or priorities; economic, educational, social, or cultural) of the communities of Lake County and beyond? How is the project innovative or unique?
c. What value does the project deliver to the Lake County tourism industry?
d. How does the project impact Lake County’s hotel and hospitality industry?
3.Interaction with the Community:

 One (1) original plus twelve (12) stapled copies of the signed application form and marketing plan (template attached). Materials should not be packaged in notebooks or binders.The marketing plan is a critical part of the application, and a missing plan will result in an ineligible application.Each stapled application should include copies of photos where relevant.
 One (1) copy of the required attachments, including:
  • Nonprofit Organizations
  • Evidence of tax exemption status from the Internal Revenue Service from applicant organization. If none, applicant can provide evidence of tax exemption status plus a letter of support from a fiscal sponsor; and
  • List of trustees or directors and chief executive and financial officers, including names and affiliations.
  • Governmental Entities
  • State the type of political subdivision or other body politic classification of your organization under the Ohio Revised Code; and
  • List of elected or appointed officials responsible for governance.
