MEMORIAL SCHOLARSHIP PROGRAM

Scholarship Application – 2017

Requirements for Application

1. The applicant must be a member of a family registered at Epiphany of the Lord Catholic Church of Katy, Texas or a member of the Knights of Columbus Council No.9759 including members of the immediate family (wife, son, or daughter) of a Knight who is a member of our council.

2. The applicant must, prior to the award of the scholarship, be accepted for Fall Semester as a FULLTIME (12 credit hours) student at an accredited junior college, an accredited fouryear college, or a post high school technical school.

3. The applicant must attach a copy of high school transcript(s) and if the applicant has attended any college or a post high school technical school, a copy of the transcript(s). Note: The transcript should be as of the last completed semester. Transcripts must be submitted with the application. They can be copies and do not need to be sealed. Applicant must also provide letters of reference from three separate sources. The references do not need to be sealed.

4. One originaland five copies of the completed application (including copies of transcripts and reference letters) must be received at the Epiphany of the Lord Church office by end of day, Monday, April 24, 2017. No applications will be accepted after this time. Incomplete packages may be rejected or subject to a penalty.

5. The applicantswill be required to attend a 15minute interview at Epiphany of the Lord Church in the Knights of Columbus room on the second floor of the PAC (Life-Teen) building. The Scholarship Committee will contact those applicants selected for interviews to confirm their interview time. The interview times will be Friday May5th from 6:00pm to 9:00pm and Saturday May 6th from 9:00am to 3:00pm.

Fill out the following charts so that the Scholarship Committee can set up a schedule. In the first and second preference columns, let us know which times you prefer within at leasta 1-hour timeframe on Friday and/or Saturday. Longer timeframes are appreciated. We will attempt to schedule your interview as close to the middle of your requested time as possible. The Scholarship Committee will schedule the interviews based on the times requested to the best of our abilities. Times may be subject to change. We will e-mail you with your time.
