Job Purpose/Summary

The post holder liaises with Marie Curie Rapid Response Nurses, Case Managers and community teams in addition to patients and families, to co-ordinate the Marie Curie Rapid Response Service which provides end of life patients with nursing care in their own homes. The post holder works within a team using a telephone network, and is supported by a computer application.

Accountabilities (Duties & Responsibilities)

  • Liaise via the telephone with NHS, local hospice and Marie Curie Cancer Care colleagues.
  • Inputting details of Marie Cure Rapid Response Service onto the computer software system and maintaining a database of contacts made.
  • Maintaining records.
  • Working as part of a team under the supervision of the Clinical Nurse Manager.
  • Taking telephone referrals from patients and families, Community Nursing Teams and other health professionals, inputting patient details and nursing care required/given onto the Marie Curie computer system.
  • Allocating Marie Curie Rapid Response Nurses to patients, to ensure continuity of care and efficient deployment of available nurses.
  • Undertake general administration of the computer system ensuring that accurate computer records are maintained of referrals, nurse allocation and contacts.
  • Assist with the administration of management reports as directed by the Clinical Nurse Manager.
  • Answer telephone enquiries from the general public and health professionals, dealing sensitively with distressed callers, seeking advice from senior colleagues where necessary.
  • Receive and relay messages promptly and accurately.
  • To provide support for colleagues in the Marie Curie Rapid Response Nursing team as required.

Additional Information

  • The Rapid Response Service was originally developed as part of the innovative Marie Curie Delivering Choice Programme in Lincolnshire. Following independent evaluation of the

service, the countywide Rapid Response Service was commissioned by NHS Lincolnshire, for which Marie Curie Cancer Care was awarded the tender to provide. The aim of the service is to provide a rapid, flexible and sensitive approach to crisis situations within palliative care that draws upon the knowledge and skills of health professionals to prevent unwanted admission for patients who wish to receive their care at home.

  • The service is operational from 3pm to 7.00am weekdays and 24 hours a day over weekends and bank holidays. In order to achieve this service provision, shifts to be worked on a rotational basis and part time hours will be considered. However, all co-ordinators will be expected to work a combination of day, night and twilight shifts.
  • There are three Rapid Response Units working on each shift, each comprising of a Registered Nurse and a Senior Health Care Assistant who travel in a pool vehicle. There is a Rapid Response Unit based in the Boston, Lincoln and Grantham areas. Each of the three Rapid Response Units available during hours of operation will be dispatched throughout the county to attend to referrals by the Rapid Response Service Co-ordinator.
