Job Title:Hepatitis C Prison Link Worker (Glasgow)
Salary:AP4 Points 27-30 / £24,294 - £26,893
Hours of work:37.5 hours per week including some weekend and evening work
Holidays: 25 days and 10 public holidays
Place of work:Waverley Care,Queens Crescent, Glasgow and in identified prison settings located in NHS Greater Glasgow and Clyde

Job summary
Waverley Care is Scotland’s leading charity offering support to people living with and affected by Hepatitis C and/or HIV.
Key Purpose
To deliver one to one assertive outreach support and to facilitate a programme of group work focusing on behaviour change and life skills to individuals living with Hepatitis C whilst in prison and at point of liberation into the community. The service will support individuals to develop personalised liberation plans pre release and will support individuals post release to remain engaged with care and treatment. . The primary focus of this service is to enable individuals to take control of their lives and their condition leading to making positive changes and supporting individual to move confidently into the community at point of liberation. Our Life Skills Programme allow participants to coach, support and motivate themselves and each other by reflecting on their lives, their achievements and strengths whilst working on particular issues, behaviours and areas that could benefit from change. The programme provides participants with factual information and practical skills, including the use cognitive behaviour life coaching tools and solution focused techniques to help participants manage their physical, mental and emotional health.
As part of the Waverley Care Hepatitis C Support Coordination Team, and in collaboration with Hepatitis C Specialist Clinical Teams in prison, acuteand community settings and with other referring partners, the post holder will:
- Deliver one to one assertive outreach support to individuals living with Hepatitis C as they move through their care pathway, whilst in prison and post liberation in the community.
- Deliver and facilitate a programme of group work to individuals living with Hepatitis C whilst in prison.
- Deliver and facilitate opportunities for one to one and group peer support, if appropriate.
Main duties and responsibilities:
- One to one support
.
- Work with individuals to create a personal liberation plan and carry out regular reviews of these plans, pre and post liberation.
- Provide one-to-one emotional support to individuals to identify and overcome barriers to improving their health and well being, pre and post liberation.
- Provide one-to-one practical support to individuals including supporting people to attend case discussions, review meetings, assessment and treatment appointments and other appointments pre and mainly post liberation.
- Facilitate group work and Life Skills Programmes and participate in the development and design of this programme to ensure that it is responsive to needs of individuals.
- Facilitate opportunities for one-to-one and group peer support.
- Networking and partnership work
- Receive referrals from the prison BBV Team, statutory and non statutory services as well as self referrals, and to record the source of referrals.
- Liaise and network with internal Waverley Care staff and with external agencies across NHS Greater Glasgow and Clyde, particularly with community based addiction nurses, Hepatitis C specialist nurses in clinical settings and other appropriate referral partners, to support individuals at point of liberation.
- Attend and contribute to the Project Steering Group for this service.
- Attend NHS Greater Glasgow and Clyde Prison Hepatitis C Planning Group meetings as instructed by the Senior Manager Glasgow.
- Publicise all aspect of the service delivery to health and social care services and to other specialist BBV services and addiction services.
- Attend and present at local and national conferences/seminars when required.
- Monitoring and Evaluation
- Routinely monitor and evaluate all aspects of the service delivery.
- Record and keep up to date records of all client interventions on the Waverley Care Service User Database.
- Provide required plans and reports to our funders and project steering group.
- Involving Others
- Ensure service user involvement in the effective design, delivery and review of the service.
- General
- Participate in team meetings, case management meetings, team training and service planning.
- Work to and adhere to all Waverley Care policies and practice guideline.
- Work to and adhere to all Scottish Prison Service (SPS) policies and practice appropriate to this service delivery.
- Attend and participate in training delivered by SPS to support the delivery of this work in the prison setting.
- Participate in supervision and annual planning.
- Identify and undertake relevant personal training, including training in Motivational Interviewing.
- Work evenings and weekends as the service demands.
