MWHS Band Booster Club Volunteer Form

Thank you for choosing to be involved with your student’s chosen activity! By filling out this form, the Volunteer Coordinator will better know how you are interested in helping. We ask that each parent volunteer two times during the school year. Volunteers are important to provide services and help to raise funds to better the program for our students!

If you are unable to volunteer your time, there will be opportunities to make various donations during the year.

MISD requires all volunteers, except MISD employees, to submit a Volunteer Information Form each year. If you are an MISD employee, please send Mr. Crofton () an email letting him know you are a district employee who wishes to volunteer.
