JOB DESCRIPTION-LICENSED CLINICAL SOCIAL

WORKER/LCSW-ISUMMARY

Under the direction of the Supervisor of Behavioral Health (LCSW) and the Clinical Administrator, the LCSW-I will be responsible for the providing the following direct services including individual couple, family and/or group therapeutic services to patients, including assessments, treatment plans and discharge planning. All LCSW- I's must have a Masters in Social Work from a accredited University and have obtained approval and a intern number from the Nevada Board of Social Workers.

ESSENTIAL DUTIES

* Perform individual therapy

*Conduct groups and lead IOP

*Perform Adult Mental Health Assessments (AUMHA) for new clients at the first meeting in which 60 minutes will be allocated to do so.

*Perform a concurrent AUMHA every six months for the client. It is the responsibility of the therapist to track expiration dates.

*Perform a LOCUS, at the first meeting with the client.

*Perform a concurrent LOCUS for each client every 90 days.. It is the responsibility of the therapist to track expiration dates.

*Perform a treatment plan with the client present at the second meeting with the client must participate in and sign the treatment plan).

*perform a updated treatment plan with the client every 90 days (client must participate in and sign the treatment plan). It is the responsibility of the therapist to track expiration dates.

*Complete a SMI determination

*schedule clients for on - going individual therapy

*Track sessions used for FFS patients only and submit a new PAR for therapy on the ninth session.

*Track current therapy PARS and resubmit a concurrent PAR for therapy 15 days prior to its expiration.

*The therapist is responsible for scheduling their own clients.

*The therapist will have a weekly schedule in which they are to see their clients for individual therapy.

*The therapist will perform a MSE at the beginning of each session and during the assessment.

* The therapist is responsible for entering daily patient progress notes in DAP format within 24 hours of service delivery.

*Therapist is responsible for attending all required staffings.

*Work in collaboration with multidisciplinary team members including external partners.

*Demonstrates cultural competency and commitment to patients

*Demonstrates commitment to the philosophy of the agency.

*Work within the scope of license making appropriate referrals when necessary.

*Develop and implement, with client, an appropriate aftercare plan prior to discharge

*Notify Clinical Director of problems and appropriate plan of action to be taken or request guidance to resolve any issues

*Maintain frequent communication with Clinical Director ensuring a united approach to treatment

*Attend necessary classes keeping abreast of treatment techniques and maintaining certification requirements
