Job summary

The post holder is responsible for the delivery of practice nursing services care to the practice population.

Supported by senior nurses within the practice, they will deliver care within the boundaries of their role,

focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and

screening activities. They will work collaboratively with the general practice team to meet the needs of patients,supporting the delivery of policy and procedures, and providing nurse leadership as required.

Key responsibilities

Clinical practice

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or

emergency situations, including initiation of effective emergency care;

Support patients to adopt health promotion strategies that encourage patients to live healthily, and

apply principles of self-care;

Deliver opportunistic health promotion using opportunities such as new-patient medicals;

Assess and care for patients presenting with uncomplicated wounds;

Support and advise women requesting information relating to family planning needs;

Support and manage health needs of women presenting for cervical cytology consultations;

Implement and participate in vaccination and immunisation programmes for both adults and children;

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad;

Promote and deliver evidence-based care for patients presenting with aural conditions.

Delivery a quality service

Recognise and work within own competence and professional code of conduct as regulated by the Nursing andMidwifery Council (NMC);

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation,policies and procedures;

Prioritise, organise and manage own workload in a manner that maintains and promotes quality;

Deliver care according to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care;

Participate in the maintenance of quality governance systems and processes across the organisation and its activities;

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvementswhere required;

In partnership with other clinical teams, collaborate on improving the quality of health care, responding to localand national policies and initiatives as appropriate;

Support and participate in shared learning across the practice and wider organisation;

Understand and apply legal policy that supports the identification of vulnerable and abused children and adults,being aware of statutory child/vulnerable adult health procedure and local guidance;

Take responsibility for own development, learning and performance including participating in clinical supervisionand acting as a positive role model.

Management of risk

Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee themonitoring, stock control and documentation of controlled drug usage according to legal requirements;

Undertake mandatory and statutory training;

Apply infection control measures within the practice according to local and national guidelines.

Utilising information

Review and process data using accurate Read codes about patients in order to ensure easy and accurate

retrieval for monitoring and audit processes;

Understand own and other’s responsibility to the individual organisation regarding the Freedom of

Information Act.

Learning and development

Disseminate learning and information gained to other team members in order to share good practice

and inform others about current and future developments;

Assess own learning needs and undertake learning as appropriate;

Make effective use of learning opportunities within and outside the workplace, evaluating their effectivenessand feeding back relevant information;

Provide an educational role to patients, carers, families and colleagues in an environment that facilitates

learning.

Minimum 2 years post registration experience

Experienced Practice Nurse with at least one year recent General Practice experience.

Management of long-term conditions.

Involvement in implementing and using protocols and clinical guidelines.

Experience of audit.

A. Salary

This will be between £21,000 to £31,000FTE (based on a 37 hour week), starting salary subject to consultation and based on relevant experience.

B. Pension

You will be entitled to contribute to the NHS pension scheme and superannuation deductions will be

made from your gross salary.

C. Holiday entitlement

This will be 25 days pro-rata plus public holidays when the practice is closed

D. Maternity leave

Statutory maternity pay (SMP) will be applied during the first year of employment, thereafter employees

are entitled to our enhanced maternity entitlement. This is two months of full pay, followed by two

months of half pay followed by SMP for the remainder of the paid period of maternity leave.

E. Paternity leave

Subject to qualifying rules, you will be entitled to two weeks fully paid paternity leave.

F. Sick leave

Your entitlement to occupational sick pay is service related and increases to two months on full pay

and two months on half pay by the third year of your employment.

G. Probationary period

There will be a three month period of mutual assessment, extended for a further three months if felt

necessary by the practice.

The practice has a staff handbook detailing all employment policies. This is available for reference for

all employees and includes grievance and disciplinary procedures.
