Event attire: jeans, BASICS long sleeved shirts, coats, gloves, scarves,
(depending on the weather), and any “festive” gear you would like to add
to show your personality!

Program cost is $25 per student, for event shirt and insurance.
Register siblings for $20 per child.

I, (GUARDIAN NAME) , am requesting to enroll my student (STUDENT NAME) in the BASICS winter glee club. I agree to the following conditions, as part of my student participating in the program:
Good behavior is expected from all participants. Any student who does not behave accordingly may not be eligible to continue participation and may be restricted from future enrollment.
In recognition of the risk of injury while participating and as consideration for the right to participate in this event, I do hereby, on behalf of my above listed participating student, waive any and all rights, claims or causes of action arising from participation, and do hereby release and forever discharge BASICS from any and all liability or damages, existing or which hereinafter may arise as a result of my student’s participation in this event.
I recognize that various photographs, video recordings, and other media will be taken during this event. I agree to grant BASICS or the media use of my student’s likeness for the purpose of promoting BASICS as deemed appropriate.
In signing, I assent that I am the parent or legal guardian of this participating student, and I agree to all of the terms and conditions above.
