Internship Preference Instructions

Please feel free to contact me and come by to talk, I encourage this. My office hours are Tuesday, Wednesday and Friday in room 2116. Pleaseemail me first to be sure I will be available if you think we will need extended time as I am also glad to make an appointment with you.

Everyone will ultimately be assigned to two internships, one of which must be primarily oriented towards treatment of adult physical dysfunction. This could include inpatient, outpatient, rehab, acute care, skilled nursing or similar settings. Hand therapy and many pediatric rotations are recommended to be taken following your general physical dysfunction experience or considered as a specialty rotation.

You may find an additional copy of this form on the OT website under Fieldwork, Student,sign in with your Gatorlink information, and then Forms.

You may only request spots that are actually offered. (ie: are listed as Available or Available and Reserved). Reservedis an internal code that means those slots are offered to UF on a first come – first serve basis. This means that the first school to request the spot with a specific student’s name is granted that internship spot. We may find out later on that they are not actually available. We can usually get the slot but this is somewhat of a gamble. Do not be swayed when you see this, but understand the process.

For your reference specialty dates are:

July 30, 2012 – September 7, 2012

October 29, 2012 – December 7, 2012

Specialties will be chosen after all 12 week assignments have been finalized.

The computer program we use dictates the format for how the lottery will be run but there will be some chances for possible 1:1 direct switching with other classmates. In order to switch you will both have to agree to this verbally and in writing. However, once placements are confirmed there will be no further opportunity for changes.Students are NEVER permitted to contact sites to arrange, change or cancel placements!

If you are interested in a specific location that is not offered on our list of available sites you may have another option. This only applies to specific locations where we DO NOT have at least one option already available to you. For instance, if there is a pediatric site available in Timbuktu and you want a physical dysfunction site there you cannot request investigation into another facility in Timbuktu. We have several blanket contracts that cover all of the facilities that a particular corporation owns. You may only look into this for an existing corporation with an existing fieldwork contract! You CANNOT just ask for a particular site, like that hospital located across the street from your parent’s home.

Go to their website and locate a facility in the area you prefer. Please try to investigate this site as much as possible and try to determine if there may be OT available at that site, BUT do not call them!

I must have the following information from you in order to pursue your request:

-Specific name of the site and the exact address

-Phone number and email, if that is available

-Dates you wish to go to this site, in order of preference if you provide more than one site

If you ask me to contact one of these sites please be sure you are truly interested in going there. I cannot call a site just to make an inquiry about availability. If they are willing to accommodate a student and I feel it would be an appropriate placement, it will be entered into the lottery system for any one of your classmates to also choose. I will be sending out periodic emails adding these new sites so please be sure to empty your inbox and be vigilant to future emails from me. Your classmates will have the option to make this one of their preferences, but you MUST include this as your number one choice.

We do have some sites that offer housing; some are free, some are at a lower cost, or may just have a housing list. Please be sure to refer to the site file for specifics. The following is a preliminary list of these:

Procedures for Checking Out Fieldwork Site Files

Location: OT staff office Room 2101. Do NOT Disturb the OT staff Hrs: Mon – Fri 8:30-4:30

Please note that Room 2101 is a private secretarial office; respect this workplace. Beth will have your files available for pick up at the desk directly in front of you as you enter the room. Please enter one at a time and make your visits timely. Return the files to Beth in this same fashion.

-An email stating your request is to be sent to bothBeth Price at and Margaret Odom . Please email several days before or early the morning you want to check out the file. Include all files you wish to check out for the day in one email.

-Include the date and time you will be in to review the files.

-Indicate the complete name of the sites you are requesting to check out. For example, HealthSouth RehabilitationHospital at Las Vegas. The location is NOT the name of the facility.

-You may check out 3 files at a time. We are doing this so that the files will also be available for your classmates. If you prefer you may review the files in small groups as well. BUT, remember to enter the office one at a time!

-The files are to remain on the second floor at all times. You may review them at the tables located between the elevators on the second floor. Be prepared to take notes, the files are not to be taken out of the building for any reason.

-If more than one person requests a file, it will be available to whoever emails first. You will be able to check out the file at a later time.

-Although we prefer that you use the email method for checkout, we do realize that you may upon occasion forget to email before coming to campusor do not have access to a computer. In this case, bring a list of three files you wish to check out with you to the office and present it to Beth being sure to follow the office courtesy outlined above.

Instructions to Complete Preference Form

Please list your preferences on the form provided in the following fashion, in order of priority, with 1 being the most favored:

  1. List up to 6 spots for ONLY TWO of the rotation dates. The rotation dates overlap so you will ultimately be assigned May and either August or September.
  1. Specialties take place in July or October and will be determined in a separate lottery after the 12 week assignments have been finalized.
  1. Be sure to indicate the internship type (outpatient, inpatient rehab, combination, pediatrics) so there is no confusion if more than one type is offered for that date at that site. Combination is used for physical dysfunction sites. It often means that it is truly a combination of acute, rehab and/or outpatient. It may also mean that the site has not specified exactly what area it will offer. They have the freedom to choose that at a later date. Refer to the site files for further details and explanations.
  1. If you list more than 12 preferences or more than 6 sites per rotation date your preferences will not be entered and you will be assigned solely by the computer.
  1. It is ok to list the same site for both of the rotations.
  1. If you decide to list fewer than 6 sites for each rotation, this is your option. However if one of these sites is not available, the computer will place you at a random site.
  1. Some outpatient sites have several locations and the placement may vary depending on the assignment made at the discretion of the site coordinator. However, they often request your input and/or preference. See the site file for possible locations.
  1. Internship sites in Gainesville are limited. Please be aware that you may only be assigned to one internship in the Gainesville area (this includes sites within one hour’s driving distance of Gainesville). Your second assignment must be outside the Gainesville area and may even be out of state. As you select your preferences,

consider this, and make your choices accordingly.

Please be aware that internship slots may be cancelled by the site at any time or that the contract may expire or we may have other contract difficulties. We will work with you on an individual basis to find a suitable replacement should the need arise. The site may change what type of internship is available, the date, and the supervisor. Housing and stipends are always subject to change and a change in these benefits will not be considered an appropriate reasonto cancel a placement. These changes/cancellations do not happen routinely nor are they predictable but they do happen every year.
