International Thespians Festival: FIELD TRIP CONSENT/ CONTRACT

I hereby give my consent for the above named student to go with the teacher or sponsor on a school field trip (International Thespian Festival which has been scheduled for the dates of June 19th thru June 26th, 2015). I hereby grant permission for school employees to secure medical services, if necessary, for my child. I understand that the school district assumes no liability in case of accident, and I hereby release the district, its Board of Trustees, employees, and agents from any claims and/or damages of any kind arising out of, or in any way connected with, the field trip.

I understand that students caught leaving the site without permission from the sponsors for any reason will face school discipline actions upon return from the festival. I understand my student is responsible for their own belonging such as but not limited money, phones, tablets, change of clothing. I understand that only breakfast will be supplied by the hotel we are staying in lunch and dinner are my and my student’s responsibility. I understand that no student may go into the room of another genders room. I understand that my student is not to pack or bring items to play tricks on each other, noise makers, drugs, or weapons of any kind.

Parent and/or guardian signature is required for a student to be released to participate in the activity.
