INSTRUCTIONS FOR MSW APPLICATION

The School of Social Work at San Francisco State University (SFSU) appreciates your interest in our Master of Social Work (MSW) Program. If you wishto apply for Fall 2017admission, please complete all steps outlined below.All applications must be complete to be considered for admission.

APPLYING TO THE SFSU SCHOOL OF SOCIAL WORK MSW PROGRAM

STEP 1: Submit the CSU Mentor Application and comply with all SFSU Graduate Studies requirements. For more information, please review the following information from Graduate Studies:

STEP 2: Submit a complete application with the School of Social Work, which must include all application components. For more information, please review the following information from the School of Social Work:

You should retain a copy of your completed application for your own records. Please include all supplemental materials (Recommendation and Verification forms) along with your MSW application in one packet.

Fall 2017 MSW Application Check List

CSU Mentor Online Application (csumentor.edu). DUE no later than December 1, 2016.

Please send your official transcripts to Graduate Studies at SF State (do NOT send them to the School of Social Work). We recommend sending transcripts to Graduate Studies as early as possible.

Three (3) Recommendation Forms (please submit the completed form and NOT a general letter of recommendation. These recommendation forms need to be in sealed/signed envelope – All three recommendation forms are DUE by December 15, 2016.

Verification Forms forEmployment and/or Volunteer Experience for each human services-related position listed on your application are DUE by December 15, 2016.

MSW Application (and separate Title IV-E application if applying for Child Welfare Stipend program with a separate Title IV-E letter of recommendation) - DUE no later than Dec 15, 2016.

Hand delivered complete packets to the School of Social Work must be received no later than

4 pm on Thursday, December 15, 2016. Do not slip envelopes containing documents under the school of social work door. Must be delivered to a person in the office, HSS 227.

Mailed applications must be postmarked no later than 11:59 pm on Thursday, December 15, 2016. Please use the following address:
