Note – This form is to be submitted with the Certificate of Satisfactory Completion of Water Service Work for the cleaning and disinfection of water storage tank. The Licensed Water Service Plumber shall submit these forms to Director, Water Supply (Network) Department within seven days of completion of the work.

I, the undersigned, hereby certify that†:-

All the above water storage tanks are in a condition fit and safe for the storage of potable water in accordance with the Public Utilities (Water Supply) Regulations and the Singapore Standard CP48 - Code of Practice for Water Services. Inspection, cleaning and disinfection of water storage tanks are carried out in accordance with Annex A of CP48.

A joint inspection on ______(date) had been carried out with the building owner or building owner’s representative, prior to certification

Water in the tanks used down to 150 to 200mm from the bottom of the tanks before the tanks are isolated and the outlet pipes shut off for cleaning and disinfection of the tank.

All downfeed and distributing pipes from the tanks had been thoroughly flushed until clean before water from the tanks was put into use and there are no leaks in the pipes and fittings at the above premises.

The attached reports from a SAC-SINGLAS accredited laboratory show that the water samples taken from every tank passed the chemical and bacteriological examinations.

No non-potable pipe running above/inside the water tanks.

Note: In Annex A of Singapore Standard CP48 - Code of Practice for Water Services, before the commencement of any cleaning and disinfection work, a joint inspection with the owner or his representative shall be carried out to check on the condition of the water storage tank (including its associated fittings such as the overflow pipes, air vents, nettings, tank manhole covers, etc and ancillary equipment). The checklist given above is meant to assist in the inspection of the water storage tank and it is by no means an exhaustive list.
