Insertion:

Insertion Checklist (audit form):

REMINDERS:

·  Complete audit at time of insertion

·  Ask inserter if they washed their hands (if visually not seen)

·  Have inserter leave enough room for Biopatch (1 ½- 2 cm)

·  STOP PROCEDURE if there is a break in technique

·  Those assisting the line insertion must wear maximum sterile barrier

Once a new line has been placed, initiate IV fluids as ordered with NEW IV fluids/tubing.

Maintenance:

Dressing Changes:

·  Gauze with transparent dressing: Change dressings every 48 hours.

·  Transparent dressing: If integrity of the dressing is maintained, the dressing must be changed every seven (7) days.

Access Devices (Invision plus valves) are to be changed with dressing changes (IV Device 2-6).

All dressings must be dated and timed, if not found upon assessment then the dressing will need to be changed.

Dressings and access devices must be changed immediately upon suspected contamination or when integrity is compromised.

All IV tubing must have a color coded label with date, time and initials. If IV tubing found without label then a new set will need to be initiated.

Continuous tubing is to be changed Q 96 hrs.

Intermittent tubing is to be changed Q 24 hrs.

No re-looping of IV tubing when disconnected; the end of the tubing must be capped off using a red cap if not in use.

Assess daily need for CVC on rounds, this includes PICC lines.

Treat partial and total occluded lumens:

Ø  Contact IV Team RN to troubleshoot.

Flushing:

Routine flushing should be performed after the following:

• Administration of blood

• Administration of incompatible medications or solutions

• Administration of medication

• Intermittent therapy

• When converting from continuous to intermittent therapies.

• After withdrawal of blood

Each lumen that is either intermittently used or not in use should be flushed as indicated in the procedure every 8 hours.

Use positive pressure flushing technique. Positive pressure flushing technique is: push a bit of solution then pause, push a bit more then pause again, and continue to flush while disconnecting from or clamping the line.

Scrub the Hub for 15 seconds with each syringe attachment!

Patient and Family Education:

Prior to insertion, education to patients and their families about CLABSI prevention should be completed. Brochures are available. Document in the Pt/Family Education in Soarian.
