Purpose

Through CSU system wide and/or CSULA site licensing agreements, the software applications listed below are available for at-home use by currently employed CSULA faculty and staff. Non-CSULA employees (e.g., ASI, UAS, foundations, institutes and centers, etc.) are NOT eligible.

Instructions

After completing this form, bring the signed copy to the ITS Help Desk, Library PW Lobby. All forms must be personally submitted.

Provisions and Acknowledgement

User is currently employed as a full-time or part-time CSULA employee.

User understands that unauthorized copying of software is an act of copyright infringement, and is subject to civil and criminal penalties.

·  User acknowledges that the above software products are copyrighted and that the software company retains all title and ownership rights to the products. User agrees not to copy or permit others to copy the products in whole or in part, including the licensing code.

·  User agrees to use the copyrighted software products in accordance with all software licensing agreements currently in effect. The right to use the software is limited to the software subscription period.

·  User agrees to use the software products under this agreement only on a computer that is owned or leased by the user, and agrees to download or install only the quantity of products specifically allowed by the licensing agreement.

·  User agrees that the software products remain under the user’s control and that resale or other transfer is explicitly prohibited.

·  User agrees not to create, attempt to create, permit or assist others to create the source code from the products furnished under this agreement. User agrees to not reverse engineer or decompile the software product(s).

User agrees to use the software products strictly for the user’s own data processing requirements and not for commercial or personal gain, sharing or any other purpose unrelated to University business.

Should the user no longer be employed by CSULA, or if advised by CSULA that the site license for the software has been terminated, the user agrees to return to CSULA or destroy immediately any and all copies in the user’s possession and to forward written notice to CSULA that the programs and materials containing the products have been destroyed or deleted from all computer libraries and storage or memory devices, including those that are no longer in use or usable by the user.
