Human Factors Training and Coaching Intervention Programme 2015

PATIENT SAFETY CHAMPIONS TRAINING PROGRAMME

Programme Aims: To improve multi-disciplinary teamwork and communication, to develop a stronger safety culture and to develop patient safety champions.

The programme is available to all of the project sites within the Safer Care Pathways in Mental Health project. Partner trusts are invited to nominate five suitable patient safety champions (PSCs) for the five-day patient safety champions human factors training and coaching, and to nominate twenty five frontline staff from the project site clinical teams for the one day general human factors introduction.

Our vision is thatpatient safety champions (PSCs) will be passionate about leading safety improvement workin their teams. Theyarehealthcare professionals who, through taking on this role, will developa good understanding of thecore human factors principles which they can take back to their workplace and implement improvements that have benefits for both patients, and the staff team. Enthusiasm and a desire to lead change for improving patient safety are a must for staff that take on this role.

Overall Programme Goals:

  1. Review of clinical team safety culture data
  2. Introduce or develop high quality briefings, performance de-briefings, structured communication and critical language use of a root cause analysis (RCA) framework and Incident decision tree for analysing errors
  3. Train all staff in an introduction to Safety and Human Factors
  4. Develop patient safety champions for Safety and Human Factors

Programme Elements;

  1. Patient safety culture survey analysis and feedback to inform the training programme
  2. Introductory human factors training days for front line staff (one day per staff member)
  3. Training and coaching sessions for patient safety champions (five days in total per patient safety champion)
  4. Evaluation of behavioural change at an individual level and at team level through a variety of feedback methods.

Training Day: Introductory human factors training day for patient safety champions

Training and Coaching Days:

Patient safety champions will attend 4 coaching workshop days throughout the programme to support the introduction of team work practices in the workplace and develop a learning culture. Through discussion and reflection of experience in the workplace new trainers can also be developed (including reflective diaries).

Coaching Themes:

Coaching Day 1 – Briefing and debriefing and SBAR – ‘right people and the right conversation’

Coaching Day 2 – Psychological safety – developing psychological safety for people to be open and willing to speak and challenge. Safety huddles, critical language, ‘stop the line’.

Coaching Day 3 – What if things go wrong? Learning from incidents and After Action Review (AAR – safety learning process)

Coaching Day 4 – Developing a just culture
