Guide to Completion

This document is intended to guide the child care service in completing the Child Care Inclusion Program Inclusion Program Application, which may be found on the webpage under “Forms and Resources: Application for Inclusion Supports.”

Important to Note:

·  One application per Home or Home-room.

·  A new application is completed/submitted:

o  Each time a new request is made for a home or homeroom, including significant changes to the circumstances.

o  Annually – (the anniversary day of the initial application)

Inclusion Request type: Check the appropriate box where:

□ Initial – The request is a new application; there are no inclusion supports in place

□ Renewal – The request is an extension to an existing support

□ Amendment – The request involves any changes (increases, decreases of attendance, #’s, etc) to an existing support: attendance, compliment of children or staff, and/ or reason for request

Section A – Child Care Service Information:

Name of child care: The incorporated name of the centre-based or family child care home

Operator/ Administrator: The name of the approved operator/provider

Licensee: The name of the person holding the license of the child care service

Street Address: The physical location of the child care service

City/ Town: The city or town in which the child care service is physically located; you may use this line as a mailing address if applicable

Postal Code: The designated code where mail would be delivered

Telephone: The number whereby regional staff of DEECD may contact the Operator or individual designated to respond to inquiries regarding inclusion.

E-Mail address: The E-Mail address whereby regional staff of DEECD may contact the Operator or individual designated to respond to inquiries regarding inclusion.

Region: The region in which the child care service license or approval certificate is held

Section B: Licensing Information:

Type of Program: A Full Time program operates 7 or more hours per day; a part-time program operates 4 or fewer hours per day

Hours of Operation: The hours of the day that are licensed or approved for children to be in attendance

Days of Operation: The days of the week that are licensed or approved for children to be in attendance

Service Capacity: The maximum number of children permitted in the home or homeroom for which you are requesting inclusion supports. The capacity number is assigned by Service NL

Service Age Range: The licensed or approved age range for a centre-based or family child care service

Administrator in Ratio: Whether or not the approved operator or family home provider is counted in a child-staff ratio

Table 1 – FCC/Homeroom Information: The certification, age-range, ratio, capacity, and the number of child care spaces filled for the morning and afternoon. For centre-based child care services this information must be completed for each and every homerooms in the centre.

For regulated family child care homes:

Family Child Care (FCC): Refers to a family home approved under an Agency or a family home individually licensed in the region

Certification level of FCC provider: The level of qualification assigned by the Association of Early Childhood Educators of Newfoundland and Labrador (AECENL), upon assessment of educational qualifications.

Certification of additional staff: The level of qualification assigned by the Association of Early Childhood Educators of Newfoundland and Labrador (AECENL), upon assessment of educational qualifications. Check N/A.

Age Range of the home: The minimum age to the maximum age that children can be to be in the home.

Child-Adult ratio: The number of children to the family home provider as determined by legislation

Capacity of the home: The maximum number permitted in the home as determined by Service NL

# of child care spaces filled: The number of children enrolled in the morning and afternoon in the family child care home

# of children enrolled with exceptionalities: The number of children enrolled with identified (diagnosed and unconfirmed) delays on one or more developmental domain

For centre-based child care services:

Homeroom (HR): Refers to the homeroom, to a maximum of six (6) homerooms, of a licensed centre-based child care service. Complete for each homeroom in the Centre

Certification level of the lead staff: The level of qualification assigned by the Association of Early Childhood Educators of Newfoundland and Labrador (AECENL), upon assessment of educational qualifications. This must be completed for each homeroom regardless of whether or not it is the room that you are applying for inclusion support.

Certification of second staff The level of qualification assigned by the Association of Early Childhood Educators of Newfoundland and Labrador (AECENL), upon assessment of educational qualifications. This must be completed for each homeroom regardless of whether or not it is the room that you are applying for inclusion support.

Certification of additional staff: The level of qualification assigned by the Association of Early Childhood Educators of Newfoundland and Labrador (AECENL), upon assessment of educational qualifications. This must be completed for each homeroom regardless of whether or not it is the room that you are applying for inclusion support

Age Range of the room: The minimum age to the maximum age that children can be in the home-room at a time

Child-Adult ratio: The number of children to the number of staff as determined by legislation

Capacity of the room: The maximum number permitted in the homeroom as determined by Service NL

# of child care spaces filled: The number of children enrolled in the morning and afternoon in the child care centre

# of children enrolled with exceptionalities: The number of children enrolled in the homeroom with identified (diagnosed and unconfirmed) delays on one or more developmental domain.

Section C: Application Details:

This section must be completed for the home/ homeroom that you are requesting inclusion support. A separate application is required for each home or home room as applications are assessed on a case by case basis of the home or homeroom selected

Table 2 – Consent and Other Supports

This section should be completed if the service is requesting support pertaining to the exceptional needs of one or more children enrolled with exceptionalities in one or more developmental domains.

Note:

·  Up to three (3) spaces allotted to considering naturally occurring proportions. In exceptional circumstances where a home/homeroom has more than three (3) children enrolled with exceptionalities, please attach as a separate document.

·  The parents and child’s full name must be indicated on the Consent to release information form. Consent between the Parent and the Inclusion Consultant will be kept at the Regional Office of the Department of Education and Early Childhood Development and a copy will be kept in the child’s file at the child care service.

Child initials and Date of birth: Initial of first name followed by initial of last name; Date of Birth follows the year/month/day format (i.e. 2016/12/25).

Consent received: The Inclusion Consultant or designated staff will only complete observations, collaborate with other supports involved, etc. where informed written consent has been obtained from the parent or legal guardian.

Date started in the service: This is entered as year/month/day format. This is the first day that the child attended the child care service, including orientation sessions. if the child has not yet started and is unable to start due to exceptionally high needs the date entered should be 0000/00/00

Current Supports in Place: Check any professionals that are involved with the child. If the child is waitlisted for service, please indicate in the “other” section.Section D: Request Details

Existing supports in place in the child care service: This includes any supports that have been approved for the child care service. If consultation supports or grants are in place for the home/ centre, the type of support (consultation, grant), approval period and any other pertinent information should be included here. You may use this section for applications that may be pending as well.

Presenting concerns: This includes all of the reasons that you are requesting inclusion supports and should be as detailed as possible.

Interactions: This includes observations on how children in the home/homeroom relate to one another; the level of engagement of adults with the children – ability to interact with the children individually, in small group and as a whole group, as well as interactions that are outside typical development. If interactions cannot be observed prior to a child starting (i.e. level of need is exceptionally high), a professional referral is required from an individual qualified to address/assess the developmental needs of the child. The professional referral must outline in detail why the child would not be able to attend without support. Every effort should be made for the child to complete orientation sessions prior to submitting an application.

Modification/ Adaptations: This includes any resources needed: Professional training, resources (Sensory materials: visual/auditory/tactile that may support the environment; assistance in identifying adaptive or assistive technology; assistance with room arrangement for accessibility.

Strategies tried to date: This includes any strategies that have been tried to address the presenting concerns including any modifications that have been made to accommodate the needs of all of the children, any success, as well as any challenges related to the modification. ECEs are trained and equipped to work with children of varying ages and abilities. In exceptional circumstances, where strategies are not effective ECEs may seek additional support to foster an inclusive environment including revising their daily schedules based on needs, participate in training on a pertinent topic, or make accommodations/ modifications to the environment/equipment/program/schedule).

Expected outcomes: This includes what you expect to enhance as a result of the request. For example, staff will be better equipped to be inclusive of children with varying abilities; programming requirements will be achieved; transitions from one environment to another will be smoother, etc.

Section E – Signature

This section must be signed by the person authorized to provide the information.

Section F Information for Submission

This includes written consent, from each parent/guardian where the child care service is requesting assistance with one or more children with exceptionalities; a professional referral where a child care service is unable to enroll one or more children in the child care service without assistance due to an exceptional need, observations completed including ABC/ frequency, running records, anecdotal reports, and the child care services’ policy on inclusive practices.
