JOB DESCRIPTION
GP SPECIALTY TRAINEE IN ENT
1.INTRODUCTION
This post will be for doctors on part of the 3 year GP Scheme and will be for 4 months. It will be based 48 hours per week in the ENT Department at Tameside General Hospital
2.MAIN PURPOSE OF THE POST
The main purpose of the post is to provide the GP trainee with ENT knowledge to fulfil his/her learning development in ENT with regards to his/her carrier as a GP.
The expected learning outcomes are outlined below.
3.THE TRUST
Tameside General Foundation Trust is an Acute Trust serving the needs of approximately 250,000 people. Residents are in an industrialised area including the eight towns which make up the Local Authority of Tameside in Greater Manchester and the rural areas around Glossop, which is within the Derbyshire Authority of High Peak.
The Trust is based at Tameside General Hospital. The hospital is located on the outskirts of the market town of Ashton-under-Lyne, the largest of the eight towns of Tameside and is situated seven miles to the east of Manchester City Centre. The hospital was granted Foundation status on 1st February 2008 and offers the following services:
Emergency Medicine (A&E); Anaesthetics; Dermatology; Ear, Nose and Throat; Adult Medicine; General Surgery; Urology; Gynaecology; Obstetrics; Oral Surgery and Medicine; Orthopaedics and Trauma; Paediatric Medicine and Surgery; Pathology; Radiology and visiting outpatient specialities which include Cardiothoracic Surgery; Neurosurgery; Ophthalmology; Plastic Surgery; Genetics and Neurology. Mental Health Services form part of and are managed by the Pennine Care NHS Trust.
There are 499 acute beds on the hospital site, including 39 beds on the Medical Assessment Unit (MAU). In recent years Tameside General Hospital has also undergone significant re-development including a purpose built Day Case Theatre and Endoscopy Unit, a new Diabetes Centre, and a new department for Older People. In April 2007, the Trust opened a satellite Renal Dialysis Unit in collaboration with Central Manchester and Manchester Children’s Trust.
The Trust is currently undergoing construction of a joint £80 million PFI project. This will see new bespoke clinical facilities for all the hospital in and out-patient services including a four-room Endoscopy unit.
4.THE DIVISION OF ENT
4.1Management Structure
Tameside Hospital NHS Foundation Trust will be the employing Trust and the successful candidate will be based at Tameside General Hospital.

There are also two ENT ST doctors and one FY1 doctor.
Mrs Trish Cavanagh, Executive Director for Clinical Services, has responsibility for the Elective Directorate. There is a monthly departmental meeting to which the appointee will be expected to contribute. Tameside General Hospital prides itself in the good relationship that exists between the medical staff and management. The successful candidate will be expected to contribute positively to the development of the ENT Department and to the hospital as a whole.
The Chief Executive is Mrs Karen James and the Trust Medical Director is currently Mr. Ryan Brendan
4.2THE ENT DEPARTMENT, TAMESIDE GENERAL HOSPITAL.
The department runs a 24 hourly service from 08:00 am Monday through to 19:00 Friday from Tameside General Hospital. Emergency care after 19:00 Friday and over the weekend and Bank Holidays is provided in conjunction with the joint collaborative working arrangements set up between both Trusts.
Daily Out-Patient clinics and theatre sessions run throughout the week at Tameside Acute Trust.
The work load includes all general ENT operations, including major ear surgeries, FESS, Facial Plastics, Head and Neck Cancer diagnostic procedures, Benign Head and Neck surgeries including parotid and submandibular gland surgeries, Laser surgeries for laryngeal pathologies and snoring etc.
There are good radiological services offering MRI Scan, CT scan and Ultrasound services. The Audiology Department is fully equipped and offers tinnitus retraining service and vestibular function test for dizzy patients and has Direct Access for hearing aids. The nurse practitioner has a dedicated ear care and allergy clinic and has an expanding role as a cancer nurse specialist.
5.FACILITIES
WARDS:
Within the Department there are 3 main wards utilised by ENT. These wards are conveniently located on the same floor as the main ENT theatre and ITU department in the Hartshead Building and one floor above the Outpatient Clinic Suite.
Adult trauma surgical ward is a mixed Adult inpatient ward, shared with Orthopaedics
Day surgical Elective unit is a mixed Adult daycase ward, shared with Orthopaedics and general surgery
Children's ward is a 21 bedded paediatric surgical ward which is shared between ENT, General Surgery and Orthopaedics
In addition, there is an 8 bedded Intensive Care Unit with 5 beds designated to ITU and 3 beds designated to Surgical High Dependency Care Unit (SHDU).
There are fully equipped treatment rooms on both the adult and Paediatric wards. In addition there are portable flexible nasendoscopes for ease of transportation and convenience for the junior doctors and a good microscope and a couch in the adult in-patient ward.
THEATRES:
The Hartshead Operating Theatre is fully equipped with Ultrapulse CO2 laser, FESS equipment and a new Zeiss microscope and backed up by full recovery services.
There is another fully equipped theatre in Stamford Building, backed up by a full recovery service, which is also utilised by ENT. There is access to ENT beds closely located to the Stamford Theatre.
OUTPATIENT SERVICES:
The ENT clinics are held in modern, well appointed and dedicated accommodation – there are three main consulting rooms, fitted with stack systems for easy visualisation and documentation of pathologies in the nasal cavities, ears, pharynx and larynx, flexible and 30 degrees and 0 degrees rigid nasal endoscopes. There is also a fully equipped treatment room. Audiology support is provided in all clinics.
RADIOLOGY: There are CT scanning and excellent ultrasound facilities on site. In addition to which on site Magnetic Resonance Imaging facilities are installed.
NURSING STAFF: All departmental nursing staff fall within the management authority of Mrs J Keogh, Divisional Nurse Manager for Elective Services, and are supported by a Matron, who has responsibility for staff and the assurance of standards of clinical nursing care. All Health Professionals within the Division are encouraged to work in a multi-disciplinary fashion.
ADMINISTRATIVE SUPPORT: The ENT team is supported by 2 full time medical secretaries as well as a full time clerical officer. Offices are equipped with computers with internet access.
6.DUTIES OF THE POST
• Attending ENT clinics.
• Care of patients pre and post op for ENT procedures on the hospital wards.
• Dealing with emergency ENT admissions.
• Preparation of patients for theatre and Liaising with anesthetics.
• Assisting in theatre (minimal exposure, to facilitate learning)
• Participation in duty rota and hand over including:
o A face-to-face handover of care must take place at every change of shifts, with communication of details about all problem patients.
o The trainee must carry out a daily ward round of all ENT patients on the ward in time to allow discharge prescriptions to be obtained; and write in the notes daily.
o Discharge and clinic summaries are to be dictated on ENT patients.
o Results relating to their firm must be seen and signed daily.
On Call
Contribute to resident rota covering ENT, and/or other associated specialty
Clinical Governance
Undertake induction and mandatory training, including completion of the Educational Agreement.
This includes completing training in basic ENT examination, the use of otoscope and ear microscope and suction clearance of external ear.
In accordance with the Educational Contract, take part in audit.
7.TEACHING/TRAINING
Tameside General Foundation Trust is committed to increasing the profile for Education and Training. The post holder will be required to participate fully in education and training events.
Weekly departmental training sessions are held every Tuesday lunch time.

8.RISK MANAGEMENT
The Trust has a Risk Management strategy. All Medical Staff are required to adhere to the principles and practice contained therein.
9.MAINTAINING MEDICAL EXCELLENCE
The Trust is committed to providing safe and effective care for patients. To ensure this there is an agreed procedure for medical staff that enables them to report quickly and confidentially, concerns about the conduct, performance or health of medical colleagues (Chief Medical Officer, December 1996). All medical staff, practising in the Trust will ensure that they are familiar with the procedure and comply.
10.INFORMATION TECHNOLOGY
Successful applicants are encouraged to proactively seek to work with the Trust’s information technology resources, to explore the full benefits of information technology in delivering improved patient care. The department has a modern computer system (Medway) which is windows based. This is linked to the rest of the Trust including the Imaging department and the Pathology laboratory for ease of request. The department has developed its own webpage on the Trust’s internet site.

12.ON CALL RESPONSIBILITES
Participation in a 1 in 3 rota, as detailed in the Job plan. (Appendix 2)
13.APPRAISAL
In line with the RCGP training scheme, the post holder will be assigned an Educational supervisor and will have regular meetings to assess his/her progress.
14.SUPPORT SERVICES
a.Psychiatric Liaison Nurse The department is supported by Psychiatric Liaison Nurses who cover the department between 09.00 a.m. to 17:00hrs, and a Mental Health Team working on a Hospital at Night service. There is a very good working relationship with the Mental Health Team.
b.Physiotherapists This is provided by the main physiotherapy department.
c.Security Officer Provided by main pool of Security Officers who are readily available and have a high profile in the department.
15.ADDITIONAL REQUIREMENTS
15.1Risk Management (HEALTH AND SAFETY)
The post holder will;
a.Help in ensuring that there is an annual Risk Management Audit in Accident and Emergency and make sure that, where necessary, an action, eradicating risk is drawn up and implemented.
b.Should help be needed in resolving risk management matters, assistance will be provided by the Consultants.
c.To provide cover for colleagues as directed by your manager.
d.Risk Management (Health & Safety)
e.You will follow risk management procedures at all times. The Risk Management procedures for the Trust and the department are kept by the departmental manager.
f.You are personally responsible for Risk Management issues in respect of yourself and your colleagues
g.If you identify a potential hazard you should report it to your manager / supervisor at once. If in doubt you should speak to your manager for guidance.
h.You must always use safety equipment provided, and report any defects to your manager. You must attend Risk Management Training as directed by your manager
i.If you are a manager or have line management responsibilities for staff or a department or area of work, you are responsible for the Risk Management issues in that area. You will ensure that there is an annual Risk Management audit in your area and make sure that where necessary, an action plan eradicating risks is drawn up and implemented.
j.Should you need help in resolving Risk Management matters, you must seek assistance from your manager.
15.2Infection Prevention and Control
a.You are personally responsible for ensuring that you protect yourself, patients, visitors and colleagues from the risks of infection associated with health care activities and the care environment.
b.You must adhere to infection prevention and control policies at all times liaising with the infection control team and acting on any instructions given.
c.You must attend regular infection prevention and control update training.
d.You should at all times promote and demonstrate good practice for the prevention and control of infection.
15.3To avoid any behaviour which discriminates against your fellow employees, or potential employees on the grounds of their sex, sexual orientation, marital status, race, religion, creed, colour, nationality, ethnic origin or disability.
15.4To safeguard at all times confidentiality of information relating to patients and staff.
15.5No Smoking Policy
The Trust operates a No Smoking Policy which states that smoking is prohibited within all Trust premises and on the site. This includes entrances, exits, cars, lease cars, car parks, pavements and walkways, areas hidden from general view and residences. As an employee of the Trust you are expected to comply with this policy, failure to do so may result in disciplinary action being taken under the Trust’s Conduct and Disciplinary Policy.
