Contract and Health Guarantee

for

Limited Registration

This contract is for Limited AKC Registration. This means that the dog will not be bred in the future and is only intended to be a pet. Below are further details on Limited Registration per the AKC web site:

"No offspring of a dog for which Limited Registration has been granted is eligible for registration. Each registration certificate for such dog shall carry notice of the limitation, and the limitation shall continue, regardless of any change of ownership, unless and until the owner(s) of the litter at birth shall apply to AKC for removal of the limitation."

"A dog registered with an AKC Limited Registration shall be ineligible to be entered in a breed competition in a licensed or member dog show. It is eligible, however, to be entered in any other licensed or member event. These events include: Obedience, Tracking, Field Trials, Hunting Tests, Herding, Lure Coursing, Agility and Earthdog."

Puppy's registered name must carry the Camelot House prefix as the first word in the name. Each litter has a theme word and the registered name must also contain that word. All AKC registration applications are sent in by the litter, not individually, so a registered name should be chosen as soon as possible but no later than 7 days after pickup. This litter’s theme word isLegacy.

Puppy's AKC Registered Name: Camelot House ______

"Buyer" means individually and collectively, the Buyer(s) identified below and their heirs, permitted assigns, and personal representatives.

Buyer Agreement

1)Buyer agrees to have the puppy examined by a veterinarian within 72 hours. If the puppy is checked by a licensed veterinarian and found to be sick due to a congenital defect, the owner shall notify the breeder and provide a written statement from a licensed veterinarian describing the puppy’s condition. The owner shall return the puppy and AKC paperwork to the breeder within three days of the examination. The breeder will replace the puppy or refund the original purchase price of the puppy provided the puppy is returned within threedays. Sick does not include parasites such as worms, Coccidia, Giardia, fleas or mites, since

these are common in dogs and puppies.

2)Buyer agrees not expose the pup to other dogs, take it to any doggie play parks, or any other place frequented by roaming dogs, including pet stores, until all of his/her immunizations are complete.

3)Buyer agrees to administer Vitamin C daily, 250 mg until 3 months of age, 500 mg from 3-9 months, then 1000 mg until 24 months. Proper exercise must be included, but no overzealous exercise or stair racing or the like.

4)There is sufficient evidence that early spay and neuter can lead to long bone overgrowth; therefore, Buyer agrees to allow puppy to reach maturity before neutering. Buyer agrees to prevent pregnancy if, in the case of a female puppy, she has a heat cycle before she is 1 year old. As a convenience to Buyer, Breeder will house a female puppy on her first cycle at low cost if given notice on day 1 of the heat cycle. Dog (male) puppy must be neutered by 18 months of age. Buyer agrees not to allow the male dog to impregnate another dog.

5)Buyer agrees this puppy will never be resold to a pet store, puppy mill, another individual or business. Buyer agrees this pup will never be surrendered to an animal shelter or animal rescue. If buyer is unable to keep the puppy for any reason, the breeder must be notified. Buyer agrees to return the puppy to Golden Retrievers of Camelot House if he/she is unable or unwilling to care for their Golden Retriever. The buyer will not receive a refund; however, the breeder agrees to place the pup in a suitable home. Buyer agrees not to give the pup to a family member or friend without prior approval from Golden Retrievers of Camelot House. If Buyer fails to comply with this, Buyer shall pay Breeder damages of $5000.00.

6)Buyer agrees to give appropriate veterinary care for the lifetime of the pup through yearly vet examinations, required vaccines, boosters and preventative healthcare. Failure to show verification of yearly vet exams and up-to-date vaccines voids the health guarantee.

7)Buyer agrees that the puppy will never be exclusively housed outside. A fenced in yard or kennel is to be provided if the pup will be housed outside a portion of the time. Adequate shelter away from the elements and proper shade must be available. The pup will never be tied, chained or placed on a pulley.

8)Buyer agrees to properly feed, water, exercise, groom, train and care for this puppy. The pup will not be neglected or abused in any manner.

9)Hip dysplasia is caused by genetics, diet, overfeeding and too much exercise at a young age. Since genetics play only about a 25% role in the problem of hip dysplasia, owners are responsible for the other 75%. This can be done by:

a)Keeping the dog at his/her ideal weight since being overweight can cause stress on a pup's soft developing bones. The ideal weight for dogs allows the ribs to be felt, but not seen. If they cannot be felt, the dog is overweight. If they can be seen, the dog is underweight.

b)Not over exercising the pup to the point of exhaustion or taking him/her for long walks, runs or jogs until 12 months of age.

c)Feeding the pup a premium dog food. We recommend a large breed dog food with meat as the first ingredient, one with a four or five star rating as can be seen on dogfoodadvisor.com.

10)Buyer agrees this puppy will only be registered with the American Kennel Club (AKC).

Breeder Agreement

1)This puppy has been properly cared for and checked by a veterinarian. The puppy has received age appropriateimmunization and has been wormed at the recommended schedule of the veterinarian. The puppy has also been microchipped. Health records will be provided when you receive your puppy.

2)Breeder is not responsible if a puppy contracts a disease once it leaves the breeder’s possession. If you suspect your puppy is deathly ill, call us. We will arrange and obtain care at our expense. If your puppy is eating, playing, and drinking, he is not likely to be ill. If he should die in the first 3 days, you must obtain an autopsy (necropsy) at your expense. We cannot be responsible for poisoning, accidents, and ingestion of objects.

3)Breeder offers a two year guarantee on any puppy found to have moderate or severely affected dysplastic hips or elbows from a hereditary reason (see "limitations of health guarantee" on page 3). This guarantee is for two years from the birth date of the puppy. The

puppy must have an x-ray done by a licensed veterinarian to confirm the diagnosis and it

must be documented by an OFA preliminaryconsultation. The x-rays must also be submitted

to the breeder for review by her veterinarian.

4)Breeder offers a one year guarantee on severe congenital heart defects(see "limitations of health guarantee" below). This guarantee is for one year from the birth date of the puppy. Puppy is to be evaluated by a licensed veterinary cardiologist and have an echocardiogram with the results sent to the breeder.

5)Breeder offers a one year guarantee against any congenital defects in the puppy causing death(see "Limitations of Health Guarantee" below), which would require documentation from a licensed veterinarian. This guarantee is for one year from the birth date of the puppy.

6)Breeder offers a one year guarantee on eyes(see "Limitations of Health Guarantee" below). If the puppy develops a visual impairment due to a hereditary condition, it must be diagnosed by a member of the AmericanCollege of Veterinary Ophthalmology. If it is determined that the puppy has a severe visual impairment, cataracts or a severe eye defect requiring corrective surgery, the results must be sent to the breeder. This guarantee is for one year from birth date of puppy.

7)Breeder does not guarantee color, temperament or adult size of the puppy.

Limitations of Health Guarantee

1)The health guarantee is for a replacement pup only and is not for a refund. There are no exceptions to this agreement. In the case that a congenital defect is found, as explained in numbers 3, 4, 5 and 6 above, the buyer will notify the breeder and provide proper documentation from a licensed veterinarian to be verified by breeder's veterinarian. The buyer will then be put on the list for a replacement pup of equal value from a future litter of the same parents or the buyer may opt to pay the difference for a pup from different parents should the cost be more than the original pup. This could take up to a year depending upon when the next litter would be born.

2)This contract is only valid between the original owner and breeder. It is nontransferable.

3)Breeder is not responsible for veterinarian bills, medications or training costs approved by the buyer.

4)Breeder is not responsible for any shipping or transportation costs.

5)This contract will be null and void for any of the following reasons:

a)Signs of abuse or neglect to the dog.

b)An overweight or underweight dog. The ideal weight for dogs allows the ribs to be felt, but not seen. If they cannot be felt, the dog is overweight. If they can be seen, the dog is underweight. Either issue can be detrimental to a dog's health.

c)Allowing this dog to be used for breeding.

d)Failure to provide documentation showing that yearly vet exams and up-to-date vaccines have been maintained on this dog.

ENFORCEMENT AND ATTORNEY'S FEES: If Buyer violates any part of this Contract and Breeder elects to take action to enforce this Contract, Buyer shall reimburse Breeder for all Breeder's attorney's fees and costs. Any legal proceedings incurred as a result of this contract or business done between the owner and the breeder will only be done in Statesville, North Carolina.
