Welcome to Pathways!
The Pathways “Rehab to Home” program at our facility offers a full rehab experience for a wide variety of adults with need for rehabilitation following a hospitalization after an injury, illness, or planned procedure. Our experienced and dedicated rehabilitation professionals, including therapists, nurses, and social services professionals, work with individuals wanting to return to a safe and independent level of function in the most cost effective way for return to home in the most time efficient manner.
Pathways professionals are equipped to meet the rehab needs of all diagnostic situations, including:
- Orthopedic incidents, such as fractures, joint replacements, degenerative joint disease
- Neurological incidents, such as stroke, brain injury, progressive disorders such as Multiple Sclerosis
- Cardiac conditions such as congestive heart failure, heart disease, heart attack, surgical intervention
- Pulmonary conditions such as emphysema, COPD, pneumonia
- Other common adult disease processes such as diabetes, Alzheimer’s, general weakness related to the aging process
Accepted Insurance/Payment Methods
Pathways at Petersen Health Care facilities accepts Medicare, Medicaid, Worker’s Compensation, and most major insurance plans.

Program Components
Pathways “Rehab to Home” offers a comprehensive rehabilitation experience provided by dedicated rehabilitation professionals “Leading the Way to Recovery” with these core services:
- Physical Therapy Physical Therapists and Physical Therapy Assistants focus on increasing the guest’s ability to safely move throughout their environment by improving general strength and balance, reducing pain, and utilizing mobility devices as prescribed by their physician.
- Occupational Therapy Occupational Therapists and Occupational Therapy Assistants focus on improving the guest’s ability to safely perform daily life tasks, including bathing and dressing, home care and basic meal prep, and community navigation, done through the use of therapeutic exercise as well as adaptive technique training.
- Speech Therapy Speech Therapists focus on improving communication and cognitive skills including expressive and receptive language, as well as assessing oral motor and swallow skills for safe dietary experiences.
- Nursing Nursing staff, including Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants focus on proper healing and recovery for optimum benefit from rehabilitation programs prescribed by the attending physician, as well as provide training for guests, family, and caregivers for medication management, dietary management, wound management, and other care specifics as needed for safe return to home.
- Social Services Social Services professionals focus on assisting the Pathways guest, family, and caregivers with additional equipment and services needed for a safe return to the home environment, including mobility equipment, additional support services such as home health, home care, or meals on wheels.

To refer someone to our Pathways “Rehab to Home” program simply call our facility and ask to speak to a Pathways Admission Specialist and let us help you in “Leading the Way to Recovery!”
