Caerphilly – 1 Grant worth £497,749
Caerphilly Teaching Local Health Board / The project will be delivered across Caerphilly county borough / The project will deliver the 'Healthy Home Tutors' model. The project will promote a joined up approach to increase awareness of healthy eating and physical activity, and achieve positive changes in attitude and behaviour for overweight and obese children aged 8-11 years old and their families across Caerphilly. / £497,749
Cardiff – 1 Grant worth £493,877
Cardiff County Council / Plasnewydd, Penylan and Canton / The project is based on the Healthy Home Tutors model and will be delivered to children aged under 12 years in specifically targeted areas of Cardiff, with a particular emphasis on BME groups. The project will be delivered either in the home or appropriate community settings and volunteers will be recruited from the target beneficiaries. The project will increase awareness of and promote positive changes in attitudes towards healthy eating and exercise and will develop strategic links in order to promote a joined up approach to nutrition, physical activity and play in Cardiff. / £493,877
Carmarthenshire – 2 Grants worth £842,334
Carmarthenshire Youth and Children's Association / Llanelli and Carmarthen / This project will deliver the Healthy Places model. Over a three year period the programme will increase awareness, healthy eating habits and levels of physical activity amongst children under the age of 12, and their families in Carmarthenshire. In so doing it will utilise exciting and innovative activities that will engage with young children and their families. 12 themed community 'festivals' will be held over three years bringing together young children from clusters of primary schools and pre-school provision within a defined area, together with the wider community, and 48 smaller-scale events will be held in the 18 primary schools within these areas. Clybiau Plant Cymru Kids Club / Based in Carmarthenshire / The project is based on the 'Healthy Friends' Model. The project will raise awareness of the value of physical activity and healthy eating through the buddying of 10/11 year olds with 7/8 year olds at out of school childcare clubs. / £425,797
Ceredigion – 1 Grant worth £454,803
Ceredigion Local Health Board / Ceredigion / This project will deliver the 'Healthy Places' model by using a participatory approach to develop and deliver festivals with healthy eating, physical activity and play themes in four areas of Ceredigion. The project will facilitate festivals on a regular basis with some linking up with existing events such as food festivals and farmers markets and others becoming new events within the communities. Alongside the main festivals, the project will develop a programme of ongoing activities that will involve whole families to reinforce behaviour change. / £454,803
Conwy - 1 Grant worth £490,243
Conwy County Borough Council, Directorate of Lifelong Learning / The project will be based in Conwy. / The project will use the Healthy Home Tutors Model. The project will train Healthy Home Tutors to promote healthy eating and physical activity of young children and families through supportive sessions with the family at home and away settings. / £490,243
Denbighshire – 1 Grant worth £338,173
Denbighshire County Council / Denbighshire / This project will deliver the Healthy Places model. The Fun with Food & Fitness Festivals (FFFF) will increase healthy eating and physical activity among children under 12 years old through a range of engaging and interesting school based and community based festivals and activities. The Festivals will target 4 areas within the County: Rhyl, Denbigh, Llysfasi and the Dee Valley. Pupils from primary schools in each area will participate in a series of fun fitness tests and complete a lifestyle questionnaire which will identify pupils physical activity and active play levels, dietary intake/dietary likes/dislikes and lifestyle choices. The school based FFFF will be used as a mechanism to launch regular community based FFFF which will showcase the local clubs and activities and promote participation. / £338,173
Merthyr Tydfil – 1 Grant worth £492,113
Gellideg Foundation Community Association / Cyfarthfa, Treharris, Aberfan and Merthyr Vale, and Dowlais wards. / This project will deliver the Healthy Places model across four wards in the Borough. Over a three year period, the Programme will deliver 150 festivals in four areas. The Programme will be responsible for implementing or supporting a minimum of six activities a week in each of the 4 areas. This programme of intense activity and opportunity will be complemented by an awareness raising campaign promoting the values of the Food Fun and Fitness Programme. The events will be varied in their themes, in the number of people participating and in their locations - however, they will all come under the umbrella of the Fun Food and Fitness Programme and seek to raise awareness of diet, nutrition and healthy exercise. A key element of the festivals will be the linkages to the allotments projects, where children will be growing vegetables and fruit. / £492,113

Newport Local Health Board / The project will be delivered across the City of Newport / This project will deliver the 'Healthy Places' model. The project will be centred around the '5 a day' and '5x60' health promotion messages and will deliver a series of festivals across Newport, that will each be accompanied by a 6 week programme of healthy eating and physical activity activities for children under 12 and their families. / £499,897
Pembrokeshire – 1 Grant worth £497,181
Pembrokeshire County Council / Tenby, Pembroke, Milford Haven, Haverfordwest, St David's, Fishguard and preseli / The project will fund a team that will deliver the Healthy Home Tutors model to children aged 0-12 years in specifically targeted areas within Pembrokeshire County Council. Six 'Personal Lifestyle Coaches' will be trained in physical activity, nutrition and play in order to work with families to increase awareness of and promote positive changes in attitudes towards healthy eating and exercise. / £497,181
Rhondda Cynon Taff – 2 Grants worth 906,692
Valley Kids / The organisation is based in Tonypandy, Rhondda Cynon Taff / This project will deliver the 'Healthy Places' model. The project will provide a programme of community festivals and events to increase awareness and improve behaviour with regard to healthy living and exercise amongst under 12's and their families in highly deprived communities of Rhondda Cynon Taff. / £413,371
Bryncynon Community Revival Strategy Ltd / Abercynon / The project will deliver the Healthy Home Tutors model to children aged 0-11 years in the ward of Abercynon in RCT. They will deliver the project by removing barriers to participation and recruiting volunteer Community Physical Activity and Nutritional Assistants who will be trained to deliver the chosen model. The project will increase awareness of and promote positive changes in attitudes towards healthy eating and exercise and will develop strategic links in order to promote a joined up approach to nutrition, physical activity and play. Activities will include improving cooking skills, family exercise taster sessions and budgeting & shopping skills. / £493,321
Swansea – 1 Grant worth £373,058
Swansea Council for Voluntary Service / Penderry, Clase, Caemawr and Morriston. / The project will fund a team that will deliver the Healthy Home Tutors model to children aged 0-12 years in specifically targeted areas of Swansea. Two healthy home tutors will deliver the service to families in two geographically targeted areas, and a Swansea-wide Healthy Home Tutor Coordinator will deliver the service to targeted families from marginalised communities. The project will increase awareness of and promote positive changes in attitude and behaviour towards healthy eating and exercise. / £373,058
Torfaen – 1 Grant worth £492, 273
Torfaen County Borough Council / The project will be based in Torfaen. / The project is based on the `Healthy Friends' model and will focus on older children (aged 10- 11 years) supervising and supporting younger children (aged 7-8 years) to increase healthy eating and physical activity during breakfast, lunch activities, and after school activities. Existing school supervisors, development workers and volunteers will provide support. The programme will also run at play scheme venues through out school holidays using play scheme staff. / £492,273
