As far as I know, I am the only one to change the forms.
The only thing I did to that form today was update my
date box AFTER I discovered it wasn't working properly.
I think it hasn't worked since I updated my main template
a few weeks ago but I just hadn't checked it until today.

Dennis - I got this email, since you are on call, I guess you will be getting a defect from the helpdesk. This is same problem as helpdesk 3932 where I put the following comment last July. Please fix it like I did and If you can figure out how this keeps happening, that would be great. thanks.

This is the same problem that was reported and fixed in defect 3932 on 2/26/2008. Somehow the <form runat="server"> beginning tag and </form> ending tags are disappearing. I asked Kathie to check it again tomorrow to make sure my fix is still in. It looks like that file may be getting overlaid with an old version somehow.
