- ProvideLocal Education Agency (LEA)or Regional Education Service Center (RESC) name,thenameofthefacilitywhereprojectistakingplaceandtheprojectnumberassignedbytheStateDepartmentof Administrative Services(DAS) intheappropriateboxes.Includethelettersuffixesindicatingprojecttypeintheprojectnumber.ProjectsgrantedrenovationstatusareidentifiedwiththesuffixRNV.
- The StateChangeOrderNumber mustbesequentialregardlessofphasing.Multiplecontractorchangeorderscanbelistedonpage2.
- ContractorChangeOrderNumber:ThisspaceisprovidedforyourownrecordkeepingpurposesifyoumaintainidentificationnumberswhichdifferfromtheStatenumbers.
- ThetotaloftheawardedpubliclybidtradecontractsshallbelistedonFORMSCG-042.COasthe“OriginalConstructionContractSum”.
- Allchangeorderssubmitted for review mustinclude an accurately completed“FORM SCG-043.CO: StateChangeOrderSummary”excelspreadsheet. AcopyofFORM SCG-043.COcanbedownloadedfromtheDAS Office of School Construction Grants & Reviewwebsite.Note:Change orders submitted which do not use theofficial version of FORM SCG-043.CO available on the DAS website will be returned as un-reviewable.
- Completethesummaryoftheconstructioncontractrevisions.Fordefinitions,seereversesideofform.
- Brieflydescribethechange,thereason the change order was required,thecoderequirements,unforeseencircumstanceorthe emergencysituationwhichledtothesubmissionofthischangeorder.Provide supporting documentation asnecessary.
- Onproject change orders which include back-charges,providethe numberoftherelatedchangeorder, to allow the Office of School Construction Grants & Review(OSCG&R) to reconcile the add/deduct back-charge.
- Checktheappropriateboxregarding change ordercertification (fully eligible, fully ineligible, partly eligible and ineligible). Note: Very few change orders are fully eligible. Refer to the FORM SCG-3050 Bulletin of Common Change Order Ineligibilities, and FORM SCG-4015 Eligible and Ineligible Items for FF&E and Technology for additional information.
- Obtainallsignaturesanddatesontheappropriatelines.Architect/Contractor’snamereferstothefirmnamedinthecontract.
- Givedetailsofthecostofthechangeorder.Lineitemsnewtothecontract,orincreasesinexistinglineitemsare to be shown aspositivenumbers.Deletionsordecreasesinexistinglineitemsareshownasnegativenumbers.
- Listandtotalthecostchangesforalleligibleitemsincludedinthischangeorderinthespacesprovided.
- Listandtotalthecostchangesforallineligibleitemsincludedinthischangeorderinthespacesprovided.
- Thetotalcostofeligibleitemsplusthetotalcostofineligibleitemsmustequalthe“CostofThisChangeOrder”reportedonthefrontpageofthisform.
- Itemizedbreakdownsarerequiredforchangeorders,includingbotheligibleandineligibleitems.Identify all repair and replacementcostsinexistingbuildingsandworkoutsideofschoolpropertylines.SeeFORM SCG-4000the IneligibleCosts andLimitedEligibleCostsWorksheet (ICW)forreference.
