FORM 2: APPLICATION FOR APPROVAL OF COURSE-BASED RESEARCH

INSTRUCTIONS:
If your course requires students to undertake research that involves human participants and protocols such as interviews, testing, observations, video, or audiotape, or other forms,please complete this application form to obtain ethics approval from the Seneca Research Ethics Board (REB).

This form should be completed by faculty membersfor student course-based research at Seneca College that involves the collection of data from human subjects by students for educational purposes; meaning, research conducted by students under the supervision of a course instructor and as part of a normal course/program curriculum.

Note: Students do not complete this form, professors do so on behalf of all the student research to be executed in a given course.
Note: Please summarize the ‘universe’ of research projects that your students will be engaging in. The REB will be requiring you to submit individual study information/consent documents for each student project (one per project group). That individual student information/consent document is a key source of information for the REB to determine whether individual projects will receive REB approval (See Part C, questions 1 and 2 below).

Submission Instructions:
The Seneca REB only accepts electronic copies of documents. Please complete all 5 parts and submit the full application along with the relevant appendices in electronic format to . Please print Part E (page 9) with original signatures, and attach the scanned copy to your submission.

You will receive an email acknowledgement of your submission within 2 business days after the submission deadline date. This email will notify you if your application is complete and has been assigned for review or if you are required to modify your submission.

☐I understand the meaning of the term “minimal risk” as defined by the TCPS II, and hereby certify that all of the research projects assigned to students will involve no more than minimal risk, and will not exceed any other limitations described in the application form.

☐Students will be provided with instruction on research ethics and will be required to produce a brief description of their research projects before the project commences and a brief summary at the conclusion (including comments relating to whether any ethics-related issues arose and how such issues were addressed) to the Seneca REB.
