FIRST LEARNING SET –PRIMARY CARE – NORTH EAST

Objectives

By the end of the meeting we will:

  • Have created a sense of joint ownership and purpose between the 4 practices in relation to the MAGIC project.
  • Have learned more about both shared decision making and MAGIC.
  • Have had the opportunity to see some Decision Support tools and discussed the various stages of a shared decision making consultation.
  • Have had a chance to discuss in detail the core components of shared decision making and begun to identify what specific areas each team might focus on.
  • Have had the chance to imagine what a MAGIC practice might look like in 18months time and begin to envisage some of the steps on the way.
  • Have discussed the measurement and rapid feedback process as part of embedding shared decision making where appropriate, and agreed on how these will be initially tried out.
  • Be clear about next steps (until next learning set) and have discussed a project plan (Project planning tool to be shared prior to meeting).

30 min / Exploration of SDM

Tease out principles / 5 mins in practice groups, then whole group discussion / Set in context of my pathway map ( hand out)
Use my analysis of steps of SDM (handout)
and include GE descriptive account as background reading

10 min / Practice doing one as a demo / Suggest we take a simple decision – probably contraceptive advice or possibly taking a holiday
Ask the group to follow the decision using the guide AND help the clinician with prompts etc.
10 min / Imagining the future / 10mins in practice teams initially – if the project had gone really well and you were really pleased with what you had achieved and a patient came into your practice in 18months time – how would they know you were a magic project
