Fan Into Flame is a weekend retreat where college students are presented with the life-changing power of the Gospel. Through prayer, sacraments and powerful teaching, we will be invited to make a personal response to Jesus as Lord and to receive prayer for a fresh release of the Holy Spirit. By giving our “yes” to God, he is able to work in us by more fully activating the powerful graces we received at Baptism and Confirmation.

Assumption of Risk/Release of Liability Form

I, ______, understand and agree that the Fan into Flame retreat onJanuary 13, 14, 15, 2012, hosted by the H.L Grant Catholic Student Center of which I am a participant, involves certain risks and that, regardless of the precautions taken by the above organization, some bodily injury may occur.

Specific risks/hazards involved in the above event include, but are not limited to the following: --Driving to and from the retreat, injuries that occur during activities, and possible allergic reactions to food or insect bites.

The likelihood of such injuries may be lessened by adhering to these safety rules or procedures: --Carpool and caravan to and from the retreat and notify Saint Paul’s Outreach of any possible allergic reactions.

Knowing this information, in consideration of my participation in the above event, I expressly and knowingly release Saint Paul’s Outreach, the H.L. Catholic Student Center and their representatives, officers, advisors, and agents; the University, its officers and employees, from any and all claims and causes of action for property damage, personal injury or death sustained by me arising out of any travel or activity conducted by or under the auspices of the above organization caused by risk associated by this activity and /or the negligence of the sponsoring group.

In addition, I understand and agree that Saint Paul’s Outreach and the H.L. Grant Catholic Student Center cannot be expected to control all of the risks articulated in this form, but may need to respond to accidents and potential emergency situations. Therefore, I hereby give my consent for any medical treatment that may be required during my participation with the understanding that the cost of any such treatment will be my responsibility.NeitherH.L. Grant Catholic Student Center nor Saint Paul’s Outreach carry any sort of medical or accident insurance for the activities mentioned, unless the participants are informed otherwise. As such, participants should review their personal insurance portfolio.

Finally, I voluntarily and knowingly agree to protect, hold harmless, and indemnify, Saint Paul’s Outreach, the H.L. Grant Catholic Student Center, the University, its officers, and employees, against all claims, demands, or causes of action for property damage, personal injury, or death, including defense costs and attorney fees arising out of my participation in the above event of the above organization.

I understand that this is a substance free retreat and agree not to bring and/or consume alcohol and/or any form of illegal substance during this retreat. I have read the above agreement and have willingly signed the same for the consideration expressed and with a full understanding of its purpose. General Information

Cost:The fee will be $35, which includes food and lodging

When:Friday Jan 13th 7:00pm - Sunday Jan 15th 1:00pm

Arrive at Eagle’s Wings BEFORE 6:30pm. It will take about an hour and forty-five minutes hours to drive tothe retreat center from San Marcos.

Dinner will NOT be provided onFriday, please eat before arriving.

What to bring:

●$5-10 For transportation to give your driver

●Clothes: Sunday Mass clothes, rec clothes, tennis shoes, enough normal clothes for Friday night, Saturday and Sunday morning, warmer clothes.

●Bible/Prayer Items

●Notebook (if you like to take a lot of notes, you will be given outlines of talks)

●Pens for notes

●Toiletries

●Towel

●Sleeping bag or blankets and pillow

2. Turn Left onto Ranch-to-Market Road 32-W (22 miles)
